An incident report form must be completed by the first responder (if applicable) or the management

(in cases where a first aider did not attend or was not present to react) if there is an injury at work.
The Human Resources department can provide the form upon request.

The individual who was wounded may fill it out on their own behalf if the event or injury was small.

The occurrence has to be accurately and completely recorded. Please type 'Not known' in the box
provided if any information is unknown.
