    Dear Ms. Sheetal Rathore

    As per your interview with us, we are pleased to offer you the position of Patient Care Therapist in our
    Organization Sunlight Healthcare Pvt. Ltd, with effect from 15th March 2022.

    Your terms of joining are as under –

     You will be on probation for a period of six months effective from your date of joining.
     As mutually discussed and agreed your CTC will be Rs.16500/- per month based on your
    Performance the same will be reviewed after 3 months.
     Your employment would be subject to the terms & conditions mentioned in your appointment
    Letter, which will be issued to you on your joining.

    On the day of your reporting you are required to submit the following for official documentation &
    verification:

    1. Accepted copy of resignation from a current employer, if any.


    2. Salary statement of last three months, if any.
    3. Originals and photocopies of Academic / Professional qualifications.
    4. Originals and photocopies of work experience/s and special achievements.
    5. Copy of PAN Card.
    6. Copy of Address Proof.
    7. Latest passport size colored photographs (#2).
