The 5 years inspection and certification of product
Prevenitor HYDRIL GK Annular 13 5/8 x 5000psi,
serial no. 151215, was performed by UPETROM according to the customer requirement specified in
contract no. GOR 4400012430 and consisting of:
1. Disassembly;
2. Inspection of parts;
3. Reassembly;
4. Hydrostatic tests;
5. Painting.
All documents issued by UPETROM, that are part of Data Book, were evaluated and their met the
specified requirements of customer contract no. GOR 4400012430.

All actions description in Report of Inspection and Actions / 08.07.2021 were done according to the
UPETROM’ technical specifications, applicable for this product.
