AFFIDAVIT OF ACKNOWLEDGEMENT OF PATERNITY

I, FREEMAN SIDEÑO y TOLOSA , 58 year old, Filipino Citizen and presently

residing at Barangay Paco, Kidapawan City, South Cotabato, after having been duly
sworn to in accordance with law, do hereby depose and say that:

I am the biological father of the child, MARK DAVID B. RONDINA (MARK

DAVID R. SIDEÑO who was born on August 10, 1988 in Isabela District Hospital,
Negros Occidental;

At the time of the said child, I was not married to the said child's biological
mother;
ARACELI BARRO RONDINA;

I hereby acknowledge my paternity/filiation to the child;

I am giving my consent for the child to use my surname and for my surname to
be reflected in the child's birth certificate;

I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever legal purpose it may serve;
