    INCIDENT REPORT

    Subject: Lending of ID
    Students Ms. Ibay Danielle with ID# 11555745, BSBA HRM student and Ms. Ibay Ma.
    Involved: Mercedes
    Narrative: On or about 1131H this date, SA Romeo B. Brobante, detailed at Lobby
    Monitoring had caught on the act Ms. Ibay Danielle with ID# 11555745, BSBA-
    HRM student, lending her ID to her sister, Ms. Ibay Ma. Mercedes to get an
    access here at SDA main lobby after she had swiped and gained entry.

    Immediately, the said students were advised by the undersigned to report at


    SDA, OSB office for proper disposition regarding the offense/violation they have
    committed.
    Action Taken Aforementioned Security Agent informed the undersigned regarding the
    incident.
    Disposition Both students were endorsed to Ms. Reimer Paragas, OSB, escorted by SA
    Romeo Obreque.
