Your Petitioner, JULIA ALCALA CONOS, 81 years old, widow, a U.S.

naturalized citizen, and with Philippine permanent address at Brgy. Sta. Rosa,
Barugo, Leyte, is invoking R.A. 9225 to seek for retention/re-acquisition of
Philppine Citizenship.

She has complied with all the requirements of R.A. 9225, and in evidence
thereof, hereby submits the following:

1. Application Form
2. Petition for Retention or Re-acquisition of Philippine Citizenship
3. Oath of Allegiance subscribed on 1 September 2004
4. 3 pieces (2”x2”) colored picture over white background (front, left and
right side views)
5. Proof of payment of application fee (original official receipts)
6. Birth Certificate (NSO authenticated copy, petitioner being born in the
Philippines)
7. Photocopy of US passport Nr. 036916955 to expire 15 Apr 09 issued in
Los Angeles
8. Certificate of Naturalization No. 24624551 issued on 19 March 1999 by
the Attorney General at San Diego, CA
9. Two (2) pieces self-addressed stamped envelope (legal size)

WHEREFORE, after satisfying all the requirements of RA 9225, it is respectfully

prayed of this Honorable Office to grant the application of your petitioner for the
retention or re-acquisition of her PHILIPPINE CITIZENSHIP, as she has in fact
returned to stay for the remainder of her life with her motherland.
