Section 1.0 Executive Summary (Logan)

1.1 Products and Services:


Goal: Provide one on one individualized care to all patients with a focus on fitness for all to use.

The fitness center is an option for all patients and anyone in the Phoenix area to continue to
progress towards their own health goals. The proposed business plan is to provide specialized
physical therapy care and provide information on musculoskeletal function and injury prevention
to bring our patients through treatment plans with proper technique to return to function outside
of the clinic and within our fitness center. The public has full access to the fitness center as well.
The plan may include advising and working with physical therapists, personal trainers, and
physicians to progress safely and adequately.

1.2 Financing:
Active System Physical Therapy grant and the business loan will be used to contribute to
personnel, building costs, physical therapy, and fitness equipment, marketing material,
technology, and miscellaneous expenses all listed in Appendix B. The co-owners will not be
given a salary until the revenue is greater than the expenses and will not receive full salary until
deemed financially able by co-owners.

The largest expense is contracted personnel and software systems for example billing
consultants, documentation software, and software for taxes. The largest initial cost will be
equipment for the therapy and fitness portions of the clinic. As Active System Physical Therapy
grows and develops further need for various consultation or software systems will be planned
and discussed by co-owners.

1.3 Mission Statement:


Active System Physical Therapy’s mission is to provide evidenced-based therapy and ongoing
fitness to restore and maintain a high quality of life for all patients. Our unique skill set and 70
combined years of experience will use evidence-based medicine for individualized treatment
plans to promote musculoskeletal function and injury prevention to return to function. Our
mission is to team up with our patients to deliver the best long-term outcomes through education

and instruction of proper techniques to bring them to their personal goals and bring them back
to function while maintaining a high quality of life.

1.4 Management Team:

1.5 Sales Forecast:


There will be an estimation of 21 patients in the first month for physical therapy services. The
goal for growth includes 50 more patients every 6 months for the first year and then 100 more
patients per 6 months after the first year. For the fitness service, it is projected in the first month
to have 1 fitness membership from a previous patient, 10 fitness memberships from non-
previous patients, and 2 people obtaining personal training services for one hour two times a
week. The goal is an increase in previous patients in the fitness services will increase by 3 more
previous patients, 2 more non-patients, and 3 more people receiving personal training services
2 days a week every 6 months. The goal of opening this business is to provide consumers with
quality physical therapy and fitness opportunities in one location.

1.6 Expansion Plan:


There is no current building expansion plan at this time. However, there are plans to expand our
personnel with the addition of front office staff, personal trainers, an accountant, and others as
co-owners of Active System Physical Therapy deems appropriate.

Section 2.0 Company Summary (Justin)

2.1 Company Ownership:


Active System Physical Therapy LLC will be co-owned and operated by Logan Reder PT, DPT,
OCS, Larson Karns DPT, PT, OCS, Kate Voisard DPT, PT, OCS, Trevor Van Loo DPT, PT,
MBA, Elliott Ledy DPT, PT, OCS, CSCS, Anna Bradley DPT, PT, OCS, and Justin O’Dowd
DPT, PT, OCS, CSCS.

2.2 Company History:


Active System Physical Therapy started as an idea while the co-owners attended graduate
school together at Central Michigan University. In order to raise appropriate funding to start the
business, each co-owner worked at various physical therapy clinics for 10 years post-
graduation. Through these experiences, they realized there was a glaring need for patients to
continue their fitness journey to sustain a high quality of life. After 10 years, the co-owners were
ready to make this dream a reality. The co-owners experiences include the following:

Larson Karns obtained her undergraduate degree from Central Michigan University in Clinical
Exercise Science with a double minor in Leadership and Spanish. She received her Doctor of
Physical Therapy degree from Central Michigan University as well. After graduation, she went to
Mexico for one year to teach English to school kids. When she returned to the United States,
she worked as a travel physical therapist where she went all over the country to work with many
different patient populations and diagnoses in a variety of settings. She fell in love with the state
of Arizona and has been working there for the past seven years at an outpatient orthopedic
clinic where she became an Orthopedic Clinical Specialist and a certified hand therapist.

Kate Voisard has her undergraduate degree in Kinesiology with a minor in Health Promotion
from Michigan State University. She also received her Doctorate Degree in Physical Therapy
from Central Michigan University. She has worked with patients with a variety of diagnoses, but
specializes in orthopedic cases that relate to the foot and ankle. She has a specialization
certification in the foot and ankle along with other courses taken for this region of the body.
Before working in Arizona, she practiced physical therapy in Utah which gave her the
experience of working with people who have high-impact injuries from adventurous activities.

Anna Bradley obtained her undergraduate degree in Kinesiology with a minor in exercise
physiology from Michigan State University. From there, she then went on to get her Doctorate in
Physical Therapy from Central Michigan University. Before coming to Arizona, she worked in
various outpatient clinics across the state of Michigan treating a wide range of orthopedic
conditions and is certified in blood flow restriction therapy.

Justin O’Dowd received his undergraduate degree in Clinical Exercise Science from Grand
Valley State University. Following that, he joined Central Michigan University’s physical therapy
program where he earned his Doctorate in Physical Therapy. Justin has worked with patients
with various diagnoses, but specializes in working with athletes utilizing his Certified Strength &
Conditioning Specialist certification.

Logan Reder received his Bachelor’s degree from Saginaw Valley State University with a major
in Exercise Science and a minor in Chemistry. He then continued to Central Michigan University
where he received his Doctorate in Physical Therapy. He has experience working with patients
with amputations and helping them transition into the use of a prosthetic device. He looks
forward to treating patients with a variety of orthopedic conditions.

Elliott Ledy received his Associates of Applied Arts degree from Grand Rapids Community
College, his Bachelor’s degree of a Kinesiology major and Psychology minor from Central
Michigan University, and his Doctorate of Physical Therapy from Central Michigan University.
He specializes in working with athletes and an orthopedic population. He is also CSCS certified
as a personal trainer.

Trevor Van Loo received his Bachelor’s degree in Exercise Science from Hope College in
Holland, MI. He then attended Central Michigan University to earn his Doctorate in Physical

Therapy. He specializes in working with general orthopedic and athletic populations and he
worked in a number of outpatient clinics in Michigan before moving to Arizona.

2.3 Company Location and Facility:


This will include independent services at the current location of 2222 N 44th St, Phoenix,
Arizona 85008. This location was chosen because golf is a very common sport since the
residents can play it all throughout the year, and Justin and Eli have significant experience with
training golfers. This multi-purpose PT clinic and gym is approximately 3,404 square feet with 4
treatment rooms, an open area for general rehabilitative purposes, as well as a large gym area
for exercise performance. The population of Phoenix, Arizona is approximately 1.65 million.1
Anticipated traffic to the clinic can be found on the Arizona Department of Transportation
website.

Section 3.0 Products and Services (Larson)

3.1 Products and Services:


The service is to use our physical therapists’ expertise to provide information on
musculoskeletal function and injury prevention to bring our patients through individualized
treatment plans with proper technique to return to function outside of the clinic. The fitness
center will allow the public and previous patients to reach their own musculoskeletal and health
goals in a center that will provide one on one personal training for more advanced individualized
exercise programs. The services may include working with physical therapists, personal trainers
and physicians to progress safely and adequately.

For use of the fitness center upon discharge of physical therapy services, patients will be
provided with our fitness center information and pricing package. The public is allowed to obtain
a membership upon interest and request. In the event of an injury at the fitness center, the state
of Arizona is a direct access state allowing the individual to walk to the clinic side to schedule an
evaluation with a physical therapist.

3.2 Competitive Comparison:


There are roughly seven other physical therapy clinics, two YMCA centers, and seven fitness
centers within a five mile radius of our clinic location in Phoenix, Arizona from an internet
search. Upon a website investigation, none of the clinics have a fitness center attached and only
one clinic mentioned an aftercare program once the patient completed their physical therapy
treatments. Only two of the seven gyms in the area offer certified personal training. This means
there is an opening to provide individualized musculoskeletal care and injury prevention
exercise programs for the best outcomes to those in the Phoenix area by clinicians with a
doctorally trained healthcare background. A patient would want the use of our fitness center
because of the relationship they developed through their time with Active System Physical
Therapy and it will allow them to make progress on their personal health goals.

3.3 Sourcing:
The customer profile is to target those who need orthopedic physical therapy and anyone who
wants to utilize the fitness center at Active System Physical Therapy. The patients that receive
physical therapy here can continue to utilize the fitness center during and after their physical
therapy services. The area physicians, massage therapists, chiropractors, and other healthcare
providers will be a source for referrals as well as recommendations of prior patients.

3.4 Clinic Layout:


See Appendix A. The plan is to have the public use the fitness center at Active System Physical
Therapy as well as patients after they graduate from physical therapy to continue their progress
and advance their health. The physical therapists on staff may use the fitness center space at
any time with their patients for the additional equipment that is not located in the clinic space.

Section 4.0 Strategic and Market Analysis (Trevor)

4.1 Outlook and Analysis:


1 in 5 Americans belong to a gym or fitness club and that number has increased by 37%
between 2008 and 2018.2 Common goals for fitness club members are to gain strength, improve
endurance/conditioning, improve flexibility and lose weight. Many of these goals go along with
the original goals of physical therapy and our gym membership can help people go beyond
medical necessity and continue working on their fitness goals as well as helping to prevent
further issues. Physical therapists have the skill and knowledge to begin working on these goals
and to help develop a plan for individuals to continue improving and working on their own.

4.2 Customer Profile:


The population of Phoenix, Arizona is 1.65 million as reported in the 2020 census.1 The median
individual income in Phoenix is $31,821 and about 89% of the population is insured.3 The
median age in Phoenix is 33.9 years.1 The customers that we will target is anyone who wants to
continue working on their fitness under the supervision or with the guidance of healthcare
professionals or to build upon the progress made in physical therapy.

4.3 Competitive Analysis:


Our main competitor will be ATI physical therapy which offers many of the same services as our
clinic. There are also a number of other smaller clinics; however, they do not offer gym
memberships or access. Our clinic will stand out and be unique by offering a small business feel
and more personal attention than the larger business chains while also offering additional
benefits such as an attached gym and separate memberships. A goal for our business will be to
coordinate and partner with local fitness programs in order to promote our business as well as
promoting health and fitness options in the area.

Section 5.0 Marketing Plan (Elliott)

5.1 Marketing Objectives:


● Initiate relationships with local health food centers, massage therapists, schools,
recreation centers and golf courses.

● Contribute to community activities and events.

● Volunteer in the community to increase awareness of Physical Therapy services.

● Visit local physician offices to spread awareness of our services.

● Initiate online marketing strategies such as a website and social media.

● Promoting a small business feel within the clinic while providing quality one on one care.

5.2 Marketing Strategies:


Initial marketing strategies will be through flyers, social media presence, and word of mouth with
existing patients. By thoroughly educating patients of the benefits of physical therapy, physical
activity and maintaining good relationships with patients, they will continue to refer friends and
family members to the clinic. Phoenix, Arizona is a big city that has plenty of potential patients
that just need to be directed to our clinic. Advertising of our low cost gym will help bring future
patients into our building that might need physical therapy later on or know someone who does.

Initiating relationships with local health food centers, massage therapists, schools, recreation
centers and golf courses. This will help increase our referral numbers while also promoting a
healthy and active lifestyle to the surrounding community.

Volunteering in the community will help spread our name to the community and allow us to give
back. It will be important to volunteer for many different types of events like special needs, food
pantries, sporting events, school physicals, etc.

Speaking to local physician offices about the different specialties our clinic offers will increase
our referral number. Pamphlets and business cards will also be distributed to these offices to
advertise.

An active online presence will help draw in patients of all ages. This will include a link on our
home website with links to redirect patients to all social media sites. All social media sites will
also include links to redirect potential patients to our home website for more information and
contact information. Social media posts will be done weekly containing information on exercise
examples, proper form, fun treatment techniques, and why people should come to our clinic.

Promoting a small business feel within the clinic while providing quality one on one care.
Providing a combined 70 years of treatment while offering one on one care will set us apart from
the big chains that could pose competition. We will do our best to keep a patient with the same
therapist through their whole treatment plan.

6.1 Corporate Organization:


The Active System Physical Therapy LLC will be operated by Logan Reder DPT, PT, OCS,
Larson Karns DPT, PT, OCS, Kate Voisard DPT, PT, OCS, Trevor Van Loo DPT, PT, MBA,
Elliott Ledy DPT, PT, OCS, CSCS, Anna Bradley DPT, PT, OCS, and Justin O’Dowd DPT, PT,
OCS, CSCS. The organization consists of each owner under contract, with Trevor being
responsible for accounting, Kate and Anna handling marketing, and Larson will be in charge of
making the initial schedule. Other auxiliary services such as scheduling changes and billing will
be performed by each contracted PT through the documentation system. Technology services
will be utilized as needed by an outside provider.

6.2 Organizational Budget:
The clinic will have contract arrangements with accounting services done by co-owner, Trevor
Van Loo, to take care of financial matters on our own through an online software (such as
QuickBooks) to save money initially. As the business grows, hiring an accountant will be
considered. Our documentation system (TheraOffice) includes scheduling software, therefore
there will be no outside scheduling fees. Initial scheduling will be done by co-owner, Larson
Karns, while other employees are permitted to make changes as needed. Building and
equipment maintenance will be performed by Logan. Marketing services will be performed by
co-owners, Kate and Anna, utilizing free websites and social media platforms. Initially, profits
from personal training services will be split amongst employees. Once the clinic is more
established, hiring out a personal trainer will be considered or the PT responsible will receive
the personal training profit. Budget for employee compensation will be $100k after we have
achieved positive income. Additionally, the budget for employment benefits will be $35,000 per
year (~$5,000 per each employee).4

6.3 Owner Biography/Employee Profiles:


Kate Voisard went to Michigan State for undergrad (go green!) and then Central Michigan
University for my doctorate in physical therapy (fire up!). In her personal life, she loves to be

outdoors. Her favorite activities include hiking, backpacking, playing/watching various sports, or
going to Lake Michigan.

Larson Karns went to Central Michigan University for her undergraduate degree and continued
at Central to earn her Doctor of Physical Therapy degree. She enjoys playing with her cat,
working out, and doing activities outdoors like playing sports for fun, going for walks, hiking,
birding, or kayaking.

Anna Bradley went to Michigan State University for her undergraduate degree and Central
Michigan University for her doctorate in physical therapy. She enjoys swimming, walking her
dog, cooking, crafting, and spending time with friends.

Logan Reder went to Saginaw Valley State University for his undergraduate degree and Central
Michigan University for his doctorate in Physical Therapy. In his free time, he enjoys swimming,
playing video games, and training with his German Shepard, Odin.

Justin O’Dowd attended Grand Valley State University for his undergraduate degree and
Central Michigan University for his doctoral degree in physical therapy. Some of his hobbies
include staying active (weight training and playing sports), and watching/cheering for his favorite
college and professional teams.

Elliott Ledy attended Grand Rapids Community college for his associates degree, Central
Michigan University for his bachelor's degree, and Central Michigan University for his Doctorate
in Physical Therapy. In his free time, he enjoys hiking, camping, playing pickup sports, and
walking his dog, Mufasa.

Section 7.0 Financial Plan (Kate)

7.1 Underlying Assumptions:


Active System Physical Therapy has been given a grant of $20,000 to help with the start-up
costs of their business and accepted for a business loan of $400,000 with a 6.5% interest rate
for the assistance of purchasing equipment, funding consulting services and personnel.5,6 This is
projected to grow by 50 more patients every 6 months for the first year and then 100 more
patients per 6 months after the first year. The goal is an increase in previous patients in the
fitness services will increase by 3 more previous patients, 2 more non-patients, and 3 more
people receiving personal training services 2 days a week every 6 months.

7.2 Revenue:
Upon opening Active System Physical Therapy, physical therapy services are projected to be
reimbursed by an average of $100 per patient based on the state average.7 There will be an
estimation of 21 patients in the first month which is a 100% increase. That will be $2,100 in
revenue from physical therapy services. Active System Physical Therapy also offers fitness and

personal training memberships. The cost of the fitness membership will be $35 for people who
have been previous patients or $45 for all other non-patients.8 There will also be personal
training services offered at $40 per hour.8 It is estimated in the first month that there will be 1
fitness membership from a previous patient x ($35) + 10 fitness memberships from non-
previous patients ($45) + 1 person obtaining personal training services for one hour two times a
week ($40). This would make $805 for the first month in revenue from the fitness services. The
goal of opening this business is to provide consumers with quality physical therapy and fitness
opportunities in one location.

7.3 Expenses:
See expenses in Appendix B. The largest expense is contracted personnel and software
systems for example billing consultants and documentation software. The largest initial cost will
be equipment for the therapy and fitness portions of the clinic. As Active System Physical
Therapy grows and develops further need for various consultation or software systems will be
planned and discussed by co-owners.

7.4 Sensitivity Analysis:


See Appendix C for the projected costs and revenue for the 1st month-5 years of opening Active
System Physical Therapy. In case of an economic downturn, global pandemic, or
underutilization of physical therapy, Active System Physical Therapy would decrease the
number of consultation services and business personnel who are a direct expense to the
business. In the chart below Active System Physical Therapy projects to start making a
cumulative net profit 26 months after opening and owners will not receive full salaries until 57
months after opening.

7.5 Source of Funds:


Active System Physical Therapy will contribute $400,000 to startup operations and reserve
funds. We will also have $20,000 from a grant to contribute to all costs listed above.

7.6 Utilization of Funding:


The grant and business loan will be used to contribute to personnel, building costs, physical
therapy and fitness equipment, marketing material, technology, and miscellaneous expenses all
listed in Appendix B. The co-owners will not be given a salary until the revenue is greater than
the expenses and will not receive full salary until deemed financially able by co-owners. Once a
positive net profit is seen this money will be reinvested into the company and distributed as
deemed appropriate by the business co-owners.
