1. Screen patients on Epic
▪ Use Rad Onc Fayette schedule to screen patients
▪ Screen each patient on the schedule
• Copy and paste pt.’s MRN into “Patient Station;” Click on
“Review” once you’re in pt.’s chart; Click on “Notes”
• MD Progress notes are the most useful – Dr. Rao and Woods for
Fayette
▪ After finding an eligible patient, write down when they’re coming into the
office so you can approach them then
2. Approach patients in office before their treatment
▪ Ask the radiation therapists to point out the pt. you’re looking for
▪ Introduce yourself to the pt. and ask if they have 5-10 minutes after their
treatment to fill out a questionnaire; if they say yes, let them know you
will find them after their treatment
3. After their treatment is finished, guide them to an open exam room
4. Facilitate informed consent process and allow them to fill out the questionnaire
▪ Go through the most important parts on the ICF, including what the study
is and why we’re doing it, it’s completely voluntary, their information
remains confidential if they choose to participate, there are no follow-up
calls or visits, they can withdraw from the study at any time, etc.
▪ Get their verbal informed consent first, then allow them to fill out the ICF
▪ Make sure they know that they can skip any questions they don’t want to
answer on the questionnaire
5. After questionnaire is complete, offer them a copy of the informed consent form
6. Stamp consent form with “ORIGINAL” stamp and write your initials and the date
underneath the stamp
7. Scan and email consent form to yourself
8. Drag form into “Financial Strain Consents 2023” folder in M drive and rename it
▪ M Drive → PHC → ORS Oncology → Coordinator Files → Financial Strain
Consents → Financial Strain Consents 2023
▪ Name of file should be “Financial Strain Verbal Consent 04.12.2023

9. Create Encounter on Epic
▪ Click on “Encounter” on top of page; Click on pt. name; Click on “New”
▪ Type of Encounter: Documentation
▪ Provider: Your name
▪ In the note, type .strain for shortcut, but if shortcut doesn’t work, you
can copy and paste the note below
▪ Accept and Sign Encounter after note is complete
“On DATE the informed consent for the Financial Strain with Cancer Study was reviewed with
YOUR NAME.

The purpose, procedures, risks, and benefits were explained to the patient and adequate time
and space was given to review the consent form. The patient was given the opportunity to ask
questions and all questions were answered. The patient stated understanding of all aspects of
the Financial Strain with Cancer Study and all study related visits and procedures.

The patient voluntarily agreed to participate in the trial and provided the verbal informed
consent.

The consent was obtained before the questionnaire with administered.”

10. Upload informed consent form to pt’s Media Manager on Epic


▪ Click on “Media Manager” on top of page; Click on pt. name; Click on
“Import”
▪ Type: Research Documentation
▪ Press “Accept”
11. Enter data on REDCap
▪ Click on Financial Strain with Cancer under “My Projects”
▪ Click on “Add/Edit Records”
▪ Click on “Add new record”
▪ Enter info
• Clinical Features info can be found under notes and surgical
pathology reports (which is found under “Results Review” tab) in
pt.’s chart on Epic
