INFORMATION TO THIRD PARTIES FOR BUSINESS PURPOSES

Given the nature of our business, some third parties, such as Clients, require Upwork
Talent Group Inc., Upwork Global Inc. or its affiliate (the “Company”) to verify and/or
disclose background reports. The Company respects individual privacy. However,
complying with such requirements is simply a reality of doing business in our industry.
Accordingly, the Company is asking you to consent to the disclosure of your
background reports by the Company to third parties by marking the box below and
signing where indicated. {If you decline to provide consent, the Company will not be
able to support engagements with you for certain accounts, but you will remain eligible
for consideration by Clients for other assignments.}
