INTRODUCTION
Civil Service hospital

The Civil Service Hospital was established in September 2008 with the support of the People's
Republic of China, which is dedicated to the treatment of current and retired civil servants and
their families. The hospital was handed over to the government of Nepal on 14 November 2008
and after five months, on 29 March 2009, outpatient services were established in the hospital.
Emergency services were added on 30 May 2009 and surgery services were added to the hospital
on 21 January 2010. The hospital is run by a seven-member hospital board under the Ministry of
General Administration.

The Civil Service Hospital is located in Minbhawan, Kathmandu. The hospital, which contains
132 beds, has the main objective of providing quality and affordable healthcare services to
former and current civil servants and their immediate family members, as well as to the general
public. Employees of the civil service receive special discounts, but there are certain rules and
guidelines to be followed. The aim of the hospital is to establish effective, transparent and
smooth hospital administration. It also focuses on recruiting and training a sufficient number of
dedicated, motivated, knowledgeable and highly qualified specialists, including doctors, nurses
and other supportive staff. In the short period of its establishment, the hospital received a very
good response from the public.

The hospital have various departments such as Internal Medicine, Anesthesiology, Family
Medicine and Emergency, General Surgery, Gynecology, Orthopedics, Radio diagnosis and
Imaging, Pathology, Mental Health, Ophthalmology, Dermatology, ENT, Dentistry, Nursing,
Pharmacy, Physiotherapy and Rehabilitation, Hospital Management, Engineering and
Maintenance, Biological Department, Hospital Security Unit, Dietary and Canteen Unit, Hospital
Store Unit, House-keeping and Laundry Unit, Outpatient and Extended Health Services Unit,
Library, Operation Theatre Unit, Emergency Unit, Medical Record Unit, IT and Communication
Unit and Social Service Unit.

OBJECTIVES OF RESEARCH

The main objectives of this research report are set out below:
1. To learn about the use of MIS in the hospital.
2. To acquire expert opinions on the use of MIS in the civil hospital.
3. To analyze the activities carried out in different sectors of the hospital.
4. To observe the information management strategies of the hospital.
5. To compare and contrast the practical and theoretical application.
6. To find out about the kind of technology used in the hospital.

METHODOLOGY

This report would not have been finalized without the collection of data and information from
different sources and people. Information was gathered through multiple sources for this report.
These are:

staff to gather information, i.e., Questionnaire and Interview.

HOSPITAL CASE

The MIDAS software was used by the Civil Service Hospital for information system
management. They have used manual technology in the past to keep patient records. It was hard
for a professional to decide. To make a decision, they have to see so many files, which was time
consuming and difficult work. So, to solve this problem, Midas software was introduced to
maintain patient records.

MIDAS

Midas is a medical tool that helps make the work environment cost-effective. The Midas
software makes it easy and efficient to manage so much patient information. It stores data that
enables doctors to plan, monitor health services and contribute to the efficiency of the delivery of
health services. The whole activity is now turned into a computerized one. The way in which the
data is recorded by Midas software:
 Patient history such as name, address, age, gender, contact number and past
medical events are recorded.
 Patients are sent to room for further diagnosis after recording their information.
 Diagnosis is recorded in the patient file because drug advice is recorded for the
disease, recommendations are recorded, and the name of the doctor is recorded.
The patient's data is also recorded in hard copy for future reasons.

The patient recording software is part of the hospital information management system. The
system must maintain the data in a safe way so that it can be accessed when necessary. Midas
software's features are as follows:

As it integrates all the data, it reduces the repetition of information. It is used to eliminate the
error caused by the handwriting being digitally managed. The use of Midas software has given
the hospital management team many advantages. They can build an environment in which they
can decide how to manage our work and organize our information in an effective and efficient
manner. It has helped to track what type of patient is increasing over a certain period of time and
helps to focus on these diseases. It helps to create worklists to identify patients with a history of
high-risk infection who are admitted to the facility. It helps to create an individualized worksheet
for the patient care plan that includes goals, results and interventions, and case manager
productivity.

Midas has two primary features, the Midas smart track and the Midas report rack. Smart track
defines a rule-based list of patients and tasks, including instructions for each task, for daily
operations and communication between them and management. Report reports without the use of
a third-party application and automatic linking of the data structure and visual manual linking
tools.

This case is connected with the use of the Management Information System in the Civil Service
Hospital Human Resource Management and Inventory Management System. In order to provide
health facilities, this health service organization utilizes different hardware and software.

USES OF INFORMATION SYSTEM IN HOSPITAL
An information system for hospital management is an element of health informatics that focuses
primarily on the administrative needs of hospitals. The Hospital Management Information
System is a comprehensive, integrated information system designed to manage all aspects of the
operation of a hospital, such as medical, administrative, financial and legal issues and
appropriate service processing. Hospital management information systems provide a common
source of information regarding the health history of a patient. The system must keep data in a
safe place and control who, in certain circumstances, can reach the data. These systems enhance
the ability of health care professionals to coordinate care by providing the health information of a
patient, visiting history at the time and location it is needed. Visual results such as X-rays, which
can be accessed by professionals, are also included in the laboratory test data of patients. Internal
and external communication between health care providers is provided by HMIS. A health
management information system is a system designed to manage the information collected and
stored in any healthcare facility. This includes the offices of physicians, private and public
clinics and hospitals. These facilities collect, store, handle and send electronic medical records
from patients.

The end goal of using health information systems is to improve patient treatment by providing
every healthcare practitioner who treats this client with the most current patient data available.
This patient data is obviously highly sensitive. Therefore, the accuracy of data collected and
patient confidentiality must be ensured by any health information system used. Medical research,
policy-making data and decision-making information are other uses of patient data in addition to
individual client treatment. Large volumes of sensitive patient data are routinely accessed,
processed or stored by health information systems. Security is crucial as a result.

The uses of the Civil Service Hospital information system are mentioned below:
 Patient Record Management System
We learned from the study that the Civil Service Hospital uses a patient record
management system to keep records and details of patient information. They record the
patient with whom the doctor and infrastructure needed to treat the patients are to be
appointed. This scheme offers high-performance management of all patient records from
admission to discharge throughout their entire cycle. It guarantees that the hospital at the
right time provides the right information to the right hand. As this system has the ability
to handle both scanned images and texts, it has made it easy for the hospital to record
patients' scanned images. This system has helped the organization to have a better patient
welfare and management system.

 Inventory Management System
The Civil Service Hospital uses the inventory management system to include inventory
tracking and tracking technologies and devices used in healthcare every day. The scheme
has helped the hospital reduce spending and keep its expenditure in line. In addition, it
helped the hospital to determine whether or not the product was damaged and whether or
not the product should be supplied to patients. It helps to eliminate waste and products
that have been damaged. It has observed significant differences in the safety of its
patients by maximizing techniques developed and applying them through business
science.

 Human resource Management System


The hospital has been helped by the Human Resource Management System to monitor
and manage staff from different locations. This system helped the hospital to supervise
the work report of staff and assist at the time of employee appraisals. It maintains
complete records of employees and designations of employees. Similarly, it records
information about employee insurance, employee leaves and views, editing payroll
systems for employees, etc. It has the capacity to easily and flexibly add workers. It has
helped to eliminate paper work in hospitals. It's easy to use this system as it is fully
customizable and scalable.

 Security Systems
Civil hospital has the systems of visitor management, intrusion detection and access
control, pharmaceutical inventory fire and life safety management and video surveillance
security systems with the help of IT security systems. To streamline and improve health
care and security operations, they have fully integrated solutions and smart tools. To keep
visitors, patients and other personnel safe, the hospital has hired security staff. It also uses
other security systems, such as the use of CCTV cameras, staff alarms and doorway
electronic access control systems.

 Ticketing System
The ticketing system has enhanced the outpatient flow at hospitals and enabled
management to organize staff, resources and patient queues effectively so that patients in
a comfortable, hassle-free environment receive the right care at the right time. This
system allows CSH to organize the entire patient flow in hospital areas, map patient
travel and manage patient wait times effectively, from patient check-in to patient calling
and appointment management. It has made it possible for the hospital to engage patients

while waiting with information displays and digital signage showing queue status to show
patients to go to the room of the required doctor.

CSH has also provided an online website through which people can obtain information
about the services of the hospital, the technologies they use and the department they
specialize in, as well as detailed information about the staff working in the organization.
It has helped patients and visitors to find out the location of the hospital, hospital opening
and closing times, doctor-required shifts and other information required.

Internet & Organization

The internet is the biggest network for computers in the world. It is a global information
infrastructure made up of millions of computers that are organized into hundreds of thousands of
smaller, local networks. The Internet is the easiest way for a company to connect with clients and
customers. Since its existence, the civil service hospital has taken away the Internet facilities
from Worldlink. It's like 24 hrs. The flow of data from service providers here is continuous.
Therefore, a medium that helps the data flow is very necessary here; this aid is provided by the
world link.

Role of Internet in HMIS

The Internet offers a range of tools and services to store and retrieve data and information, as
well as to communicate it to the world. The use of the Internet as a technology for information
management offers great opportunities for new ways to work with information.

The internet offers a variety of services for the storage and retrieval of data and information and
for the worldwide communication of that information. The use of the Internet as a technology for
information management offers great possibilities for new ways of working with information.

The internet plays a vital role in data collection, as in the context of civil hospitals. The data that
has been gathered in the database appears when the data is entered into the computer. As per the
patient's appointment, the patient record is gathered with the required doctor and treatment
information in the department concerned. The civil hospital also provides home service, as stated

by the authorities, i.e. the patient's report can be viewed by remaining at home. The hospital
provides a password and the user can log in to fetch data with the help of the Internet.

Online Ticketing Service in the Hospital

With the invention of digital payment, for several utility payments in Nepal, long queues/lines
have been outdated. People still think about long waiting in lines when they were to take the
OPD tickets at Government hospitals. With a new system in place, the issue of long waiting for
the patients and family will resolve. This online OPD ticket buying facility from the Civil
Service Hospital has started to be provided by Prabhu pay. People don't need to visit the hospital
first now just to buy the tickets. They can take the tickets from their own home online.
To sum up, the Civil Service Hospital has launched online ticket bookings for Prabhu Pay
Mobile Wallet patients visiting the Civil Service Hospital in Kathmandu's Outpatient
Departments (OPD). This system is introduced for the first time at the Civil Service Tickets can
be booked two days before the date of the visit.

The implementation of internet ticketing is the efficient use of the internet in civil hospitals. With
the invention of digital payment, the hospital has arranged to take online tickets from Shrawan
22, 2076, long queues for several payment utilities in Nepal have been ousted. When they were
to take the OPD tickets at government hospitals, people still think about long waiting in lines.
The issue of a long wait for patients and families will end with a new system in place. The
program has been launched with the IME pay collaboration. The also stated that it is the first
time in Nepal that OPD tickets can be purchased online, as half of the work has to be done
manually, this process has not been fully computerized. Online ticketing is a hospital connection
framework in which the counter-based OPD registration and appointment system has been
digitalized using the Hospital Management Information System (HMIS).

How do you buy online ticket?

You need to download the Prabhu Pay App to purchase the hospital OPD ticket. We can follow
following steps:
 Go to the Prabhu pay app and use your ID to log in. Make sure that enough
balance has been loaded to purchase the OPD ticket.
 Click the icon for a hospital.
 Select an old or new patient, then. You need to fill in your details, such as billing
mode, department, date and doctor, if you are a new patient.

 It will go to the next page after prompting the service charge of Rs. 100, where
you have to fill in your information such as name, DOB and more.
 Then, for consultation, you must pay the amount. You will receive a letter of
confirmation and you will be successful in purchasing the ticket.

ANTI-VIRUS FOR INFORMATION SECURITY
1. Firewall
In computing, a firewall is a network security system based on predetermined security rules that
monitors and controls incoming and outgoing network traffic. Typically, a firewall establishes a
barrier between a trusted internal network and an external network that is not trusted, such as the
Internet. Often, firewalls are classified as either network firewalls or firewalls based on the host.
Network firewalls filter traffic and run-on network hardware between two or more networks. On
host computers, host-based firewalls run and monitor network traffic in and out of those devices.

Generally, firewalls are classified as network-based or host-based. Network-based firewalls are


positioned on LAN, WAN, and intranet gateway computers. They are either software devices
running on general-purpose hardware, or firewall computer appliances based on hardware.
Firewall appliances may also offer other functionality to the internal network they protect, such
as acting as a DHCP or VPN server for that network. Host-based firewalls are placed on the host
itself and the network traffic in and out of those machines is controlled. A daemon or service
may be a host-based firewall as part of the operating system or as part of an agent application,
such as endpoint security or protection. Each one has benefits and drawbacks. Each has a role in
layered security, however. Depending on where communication originates, where it is
intercepted, and the state of communication being traced, firewalls also vary in type.

Bandwidth Management, Load Balancing Multiple Link Management, and Gateway Failover
over a single platform include Cyberoam network security appliances. Visibility and control over
2000+ key applications are offered by Cyberoam. It offers complete visibility of which
applications, regardless of their ports and protocols, are accessed within the organization and by
which user. This stops sophisticated application-layer threats, ensuring Application Security,
right at the network perimeter. According to our research, the major antivirus software used in
the Civil Service Hospital to maintain information security is firewall and Cyberoam.

SWOT ANALYSIS

SWOT Analysis is an examination and assessment of the internal strengths and weaknesses of an
organization, its growth and improvement opportunities, and the threats to its survival and work
posed by the external environment. The SWOT analysis is a preliminary exploratory tool that
enables the hospital to examine and evaluate its internal and external environment and the
influential factors associated with it. In their decision-making process, this tool helps to take
steps to improve and increase the performance of the organization and the capacity for quality
work. For hospitals, a SWOT analysis is helpful as it helps to concentrate your marketing in
areas with the strongest benefits.

At present, the information system for hospital management used by civil hospitals has the
following strengths, weaknesses, possibilities and threats:

A. STRENGTHS
The implementation of the hospital management system has provided the hospital with various
benefits that enhance the quality and efficiency of the service. Below, some of the strengths are
listed:

Huge storage of hospital data


For various reasons, hundreds of people visit hospitals on a daily basis. The biological tests and
consulting diagnosis made by physicians have to be stored as case files under patients' names.
The Hospital Information System enables hospitals to organize, format, process and store such
records in a systematic way. All the necessary patient data is included in the hospital database.
Doctors can have access to the history of the disease, test results, prescribed treatment without
much delay to make an accurate diagnosis and monitor the health of the patient. This allows for
lower risks of errors.

Staff interaction
For better coordination and teamwork, it is essential to engage all of your employees. They do
not have to make special requests and wait for an answer for a long time. Each specialist will be
in charge of a certain stage of the process and can share results with peers with just one click.

Hospital authorities are able to manage their resources available, analyze staff work, reduce
downtime of equipment, optimize the supply chain, etc. Another fact to mention is that, instead

The work is well planned and organized at the hospital. The information will be properly stored
in data stores, which will help to retrieve and store information. The treatment process can be
less stressful, since the Hospital Information System is patient-oriented. There is more time for
doctors to examine and interact with patients.

Accuracy and Reliability


The accuracy level will be higher in the proposed system. All operations will be carried out
properly and systematically. It makes sure that whatever data comes from the center is accurate.
Proper storage of information is the reason for the system's increased reliability.

The Civil Service Hospital's strengths in the use of information technology and systems are
mentioned above.

B. WEAKNESSES
Weaknesses are those variables that hinder the ability to work and have a negative impact on a
hospital information system's performance.

 The information system for hospital management is expensive to set up and


maintain.
 In the information system, there is resistance to change and gradation.
 Security issues may occur as the system is more efficient and user-friendly.
 The information system is not standardized according to the hospital's needs.
 The report is either complete or filled with incorrect and incomplete information.
 In this system, effectiveness is also an issue, which means that the top
management team can be fired at any time and the new team can change company
policy that affects overall company decisions and the interactions of employees.
 The inherent problems of using computerized systems include power failure,
computer viruses and hackers.
 In the civil hospital context, the HMIS does not provide cloud computing at an
effective rate.
 For making non-programmed decision-making, HMIS is less useful. These types
of decisions are not routine and therefore require information that may not be
available to the executive from the existing HMIS.
 The quality of the inputs and processes is governed by the quality of the HMIS
outputs. The HMIS may therefore turn out to be less productive in the case of
ineffective input.

C. OPPORTUNITIES
Opportunities are external situations which present opportunities for growth or improvement
for a company:

 For the past several years, the number of software users has increased rapidly and is
expected to increase more because of changing government rules. For HMIS, this can
be taken as a major opportunity to achieve more potential leads and expand its
customers.
 Similarly, new technological trends are further supporting its service sector expansion.
As a leader in software, Midas technology can cope with value-added services and
strengthen its relationship with the increasing amount of software that uses devices
such as computers, printers, and other computing devices.

 Civil Hospital can upgrade online services for customers with the strategic use of
HMIS and enhance customer satisfaction. Because of the proper use of information

technology and information systems, the development of genuine websites and mobile
apps like other hospitals offers the civil hospital a competitive advantage. Hospitals
must therefore use management information system features to provide extended
patient services and keep them as loyal customers until the future.
 Hospitals and their management cannot operate well without the Hospital
Management Information System in today's highly competitive environment. The
hospital must therefore seize opportunities by scanning its external environment and
making proper use of HMIS.

D. THREATS
Threats are those external factors that are considered potential risks or hazards that could harm
the quality of healthcare organizations' work and performance.

 Security, as with any IT system, is a major threat to HMIS. All of the information is
on the network. It can turn any vulnerability or loophole into a possible security flaw.
Information system security is affected by security threats such as unauthorized
access (hackers and crackers), computer viruses, theft, sabotage, vandalism, accidents
and natural calamities. Very careful attention should be paid to this aspect of the
system.
 The design of the data can also prove to be a threat if the design is not flexible enough
to satisfy the requests for change. HMIS is a management support scheme. The
modification of the HMIS may involve any change in management. The system
should be sufficiently capable of handling such changes.
 Natural threats can turn out to be devastating if the system does not back up. For even
the smallest part of the HMIS, a complete backup policy and business continuity plan
should be in place.
 Without human wisdom, HMIS would be nothing. It can be misused if these systems
or the information about the system falls into the wrong hands. This is one of HMIS's
greatest threats.
 The shortage of human resources and untrained employees could be a threat to the
health organization because they may not have the knowledge and practice to work
with information management tools.

TECHNOLOGY GAP ANALYSIS

Compared to the technology used in the past, the technology used in health services has now
gone far ahead. Everything is digitalized and most of the operational system of hospitals are
carried out through Information Technology. In line with the latest technological advances,
hospital management can enhance the quality of care provided and the facilities available.
Healthcare services can upgrade and improve the quality of their existing services and facilities
by using gap analysis arrangements, which will eventually improve the patient experience.

A gap analysis is a management tool to foresee the areas of opportunity that can be implemented
by management to achieve their organizational objectives. This is done by evaluating the current
state or stage of the undertaking concerned by taking into account all the relevant facts and
figures and comparing them with the future or the targeted stage. The actual condition between
the present position and the future projected state is denoted by 'Gap.' Major analysis of the
technological gap related to health facilities:

3. Health care professional education
4. Patient education
5. Automated data access

CONCLUSION

For any hospital, the Hospital Management Information System is designed to replace their
existing paper-based manual system. The Hospital Information System includes patient
registration, data storage in the system, computerized pharmacy billing, and laboratories. HMIS
aims to computerize hospital management, which is simple, fast, and cost-effective and user-
friendly. The Information System for Hospital Management offers the advantages of streamlined
operations, improved administration & control, superior patient care, strict cost control and
increased profitability. The software has the ability to provide each patient with a unique ID and
stores the details of each patient and staff. The use of MIDAS software has thus helped to
manage patient information at the Civil Hospital. The use of MIDAS software has decreased the
time required to record data considerably. This has helped to record and evaluate the data in the
most efficient and effective manner. Most importantly, MIDAS helped to record and recover
information in no time.

They have different departments, such as Emergency, General Surgery, Gynecology, Radio
Diagnosis and Imaging, Orthopedics, Pathology, and many more, according to the departmental
work, providing patients with health care. Only through the efficient use of the management
information system can smooth operation be achieved. In order to cure their patients, they use
advanced information system technologies and tools. The implementation of the information
system for hospital management has helped to store all kinds of records, provide coordination,
implement policies, enhance day-to-day operations, make decisions, organize the supply chain,

RECOMMENDATIONS & SUGGESTIONS

In the field of information technology and systems, the Civil Service Hospital has both
positive and negative aspects. The hospital needs to maintain quality management in order
to be more successful. The hospital needs to make full use of HMIS to take advantage of the
competitive benefits and to minimize external risks and threats.

In terms of observation, following a field visit to the hospital and observing the use of HMIS
in the Civil Hospital, we recommend the following aspects:
 In order to get more and more advantages, it needs to update its MIS often.
 The use of information systems and information technology should be made faster,
more efficient and more reliable.
 It should focus more on hospital leadership.
 There should be feature to provide friendly environment to the visitor with the use of
technology.
 From time to time, the record should be managed and checked to find out whether or
not any data or records are misplaced by central authority control systems.

To sum up, if the hospital works more on updating technologies, managing crowds, and updating
the Management Information System, then the hospital can be more successful and run
efficiently and systematically.
