PURPOSE The Procedure outlines the general Risk Management process that PT. Tiga Pilar Energi use to identify hazards in the workplace and to identify and implement control measures to minimize rizk. This document covers the basic methods of hazard and tisk identification as described in this document 5 steps to Risk Asessment 3. DUTIES AND RESPONSIBILITIES Risk Asessment will be performed by those involved in carrying out of the activity with the risk assessment being signed off by the PT. Tiga Pilar Energi Production Manager. INSTRUCTION PROCEDURE 5.1. General Information The Company has a statutory obligation to eliminate, reduce or control risk to their employee and to others in so far as is Reasonably Practical ‘The interpretation of reasonably Practical means that the effort required to remove a hazard (in terms of time, money, effect or trouble) need only be proportional to the risk related to the hazard. In practical terms this has led to the reference “As low as reasonably Practicable” (ALARP) where the risk related to a hazard should be controlled and minimized. ‘The processes to achieve this are listed below in order of preference. Elimination of the risk (by using other processes) Reduction of the risk (modifications) Controling the exposure to the risk (barriers, quards) Provision of appropriate Personal Protective Equipment (PPE) | Number of Document TPE-K3L-PR-14 Revision Number : 00 Date of Release 17 September 2018 | Page aoe] 53, In cases where the risk cannot be eliminated, reduced or controlled to ALARP ans some risk remain PT, Tiga Pilar Energi management must decide whether these risk are acceptable. Risk assessment will also include asessments required by other requlations such as manual handling and noise. Definitions A hazard can be defined as something with the inherent potential to cause harm, injury to personnel or damage to plant/ equipment-infrastructure. Risk can be defined as the likehood of the abouve arising from a hazard, Examples of hazard and the associated risk include ; * Handling of chemical substances — there may be a risk of exposure to the chemical + Walking upon floor surface — there may be arisk of slips, trips or fails + Climbing up or down ladders ~ there may be a risk of falling from, or collapse of the ladder * Operating an abrasive wheel ~ there may be a risk of contact with, or explosion of, the abrasive wheel ¢ Use of electrical equipment - there may be a risk of electrical shocks or buns * Use of cranes ~ there may be a risk of overloading or failure of crane therefore damage to equipment/ property ‘The exient of a risk can only be quantified by an examination of the adequancy of any existing control mesures. In the first instance above, factors that would greatly increase risk include use of unlabelled containers, lack of employee training and poor ambient lighting. ‘Those particular risk can be effectively reduced by means of control measures such as ensuring that containersare properly labelled, that employee have sufficient training and that lighting is suitable for the task. Five Steps To Risk Asessment ‘The following examples are aimed at assisting and guiding in the identification of hazard that could potentially result in significant harm and which may exist within the workplace. 5.3.1. Newly hired personnel Cleaners Office Staff Workshop Staff Contractors Clients Building / Material Equipment / Machinery Pay particular attention to ; Staff with disabilities In-experience staff Visitors Lone workers Step 3 ~ Is The Risk Adequately Controlled Are precautions already taken against the risk from hazards listed ? For example are the following provided ? + Adequate information, instruction, training ? # Adequate PMS or procedure ? Do the existing precautions ; + Meet the standards set by legistlation ? + Comply with a recognized industry standart ? * Represent good practice ? + Reduce risk as fer as reasonably practicable ? If so, then the tisk are adequately controlled, but you need to indicated the precautions you have in place. You may refer to procedures, manuals company rules giving this informations, Step 4 - What Further Action Is Required to Control The Risk ? What more could reasonably be done about those risk that are found not to be adequately controlled ? Priority must be given to those risk that effect large numbers of people and/ or could result in serious harm. The principles below should be applied when taking further action, if possible in the following order ; » Remove the risk completely = Try a less risk option = Prevent acess to the hazard (eg. Shift) » sue PPE Step 5 ~ The Asessment Must Be Reviewed/ Revised When Necessry At sometimes new machines, subtances and procedures may be introduced which could lead to new hazard. If there is any significant change. The assessment must take account of the new hazard. If any case, it is good practice to review assessment from time to time. ‘This should not be done for every trial change, or still more, for each new job, but when a new job introduce significant hazards. 5.4. The Process The process described below can apply for each hazard. This process is documented on the risk assessment from available electronically or from the HSE Department, Step 1 Select the area, job or activity where y=the hazard are to be assessed Identify the hazard associated with each activity. Step 2 Ask The Question How could any injury, illness, damage or other loss result from this hazard? Note: ‘There may be activities which in themselves are not especially hazardous, however, when combined with other activities, others dangers may become apparent. The identification of such hazards requires a throught examination of the methods of work to be employed on that particular task. Step 3 ‘At this stage in the process you will have a list of hazard that could result in injury illness damage or other loss. Ask the following questions using the table below to establish What type of injury or loss could occure ? (A potential severity rating) Jow Likely is it to happen ? List the control measures that need to be taken to either eliminate reduce or control the identified hazards then enter the residual risk rating (RRR) in the last column. Blank Risk Asessment forms are available from the HSE Department 5.5. Implementation The Process Each identified hazard need needs to be processed so a decision is made as to whether ; Injury, illness, damage or other loss could result from it and if so what action is to be taken to eliminate, reduce or control the tisk. This is decision making process and must be carried out in a systematic manner. At the same time there is an opportunity to “Calculate The Risk” and so determine the relative seriousness of each hazard (j.e as compared to each others) The score is not to decide whether to act upon the risk posed by the hazard.
