The Salvation Army Toronto Grace Health Centre (TGHC) provides medically complex, and specialized care and
services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care

PHYSIOTHERAPIST – FULL TIME

Position Summary:
The Physiotherapist will be 0.4 in patient unit and 0.6 Pulmonary Rehab Clinic. Reporting to the Patient Care Manager,
Physiotherapist is responsible for the provision of holistic patient centered care in accordance with the standards of
practice of the College of Physiotherapists of Ontario. The Physiotherapist is responsible for assessment, implementation
and evaluation of treatment interventions, discharge planning and/or other services for assigned patient care unit(s)
and/or programs.

TGHC Post-Acute Care Rehabilitation (PACR) and Complex Continuing Care (CCC) programs provide care for individuals
with medically complex conditions and multifaceted chronic diseases who require integrated, comprehensive, low-
tolerance, and long-duration rehab interventions. The aim of the service is to assist the patient to achieve the optimum
level of mobility and physical functioning in order to improve quality of life.

partnerships with patients, families and health care professionals to ensure optimal care delivery.

Responsibilities In-Patient Unit:


• Assumes responsibility and accountability for the provision of patient care utilizing current physiotherapy best
practices guidelines and the principles of patient-centered care, in accordance with the standards of practice,
regulations, and ethics of the College of Physiotherapists of Ontario.
• Develops, implements, and modifies the individualized care plan which is goal-directed, evidence-based and
outcome-oriented, based on physiotherapy assessment findings, response to treatment, and patient goals.
• Evaluates the individualized care plan through discussion with the patient and/or family and in collaboration with
other members of the interdisciplinary team on an ongoing basis.
• Supervises Physiotherapist Assistants and assigns interventions to the Assistants based on their knowledge,
judgment, and skill level while retaining overall accountability for care given in accordance with the College of
Physiotherapists of Ontario standards.
• Provides supervision to Physiotherapy students and volunteers in accordance with the Standards of the College
of Physiotherapists of Ontario.
• Acts as resource and advocates for patients and families and colleagues.
• Performs other duties as assigned.

Responsibilities Pulmonary Rehab Clinic:


• In-person MSK and cardiorespiratory assessments and discharges in collaboration with RRT using
standardized outcome measures such as the 6MWT
• Ability to complete a 6MWT is an asset
• Provide a personalized exercise program for virtual rehab
• Complete all electronic and written documentation in a timely manner
• There may be the opportunity to provide group virtual rehab in the future where the candidate should
progress the program as appropriate
• Comfortable using Zoom and planning virtual classes (to coordinate with external lecturers, creating
schedules, creating Zoom meetings, sending out links, assisting patients with troubleshooting etc.)
• Report findings to Respirologists
• Participate in team meetings and initiatives
• Other administrative tasks as needed
• Rostered for O2 titration – required

Minimum Qualifications:
• Completion of a Bachelor’s of Science Degree in Physiotherapy or recognized equivalent.
• Registration with the College of Physiotherapists of Ontario in good standing.
• Current Basic Cardiopulmonary Life Saving course.
• Demonstrated excellent communication and interpersonal skills.
• Demonstrated critical thinking skills/problem-solving skills.
• Two years inpatient hospital experience in physiotherapy practice preferred.
• Previous experience working with adults with complex needs.
• Strong verbal and written communication skills with team members including Respirologists, family
physicians, other RRTs, and PTs
• Experience in the assessment and treatment of ABI CCC, critical care, cardio-respiratory and/or ICU, acute
neurological and orthopaedic, general medicine, experience an asset.
• Complies with the standards, codes and regulations of the College of Physiotherapists of Ontario and with
the legislation in the Regulated Health Professions Act.
• Excellent time management skills and demonstrated ability to perform in a fast-paced environment with
minimal supervision.
• Proficient in Microsoft Excel and Adobe PDF. Able to learn new software and documentation platforms
quickly.
• Detail oriented and able to work independently
• Demonstrated computer skills knowledge of MS Word, Excel, Power Point and Outlook
• Excellent attendance record.
• Must have the physical strength, flexibility and endurance to perform the role as determined by the
physical needs of the position based on assessment conducted by Occupational Health Services.
