JOB TITLE: Clinical Fellow FY2 Level in Local Services

JOB SUMMARY

This is an exciting opportunity for a Clinical Fellow FY2 to join our Stroke Medicine team at
Southend Hospital, part of Mid and South Essex NHS Foundation Trust. Our Clinical Fellow
(FY2 level) posts are structured, trainee centered with flexibility, competency assessed,
service based and quality assured. Our Clinical Fellow Posts have been designed to work in
conjunction with the FY2 Training Posts.

The Stroke department at Southend Hospital is recognised as a regional centre of


excellence, it provides a caring and comprehensive service for all aspects of stroke treatment
for the population of Southend and Essex.

Southend hospital has both a comprehensive stroke thrombolysis service, and also a best-
endeavour stroke thrombectomy service. We have both Carotid Surgery and Stenting
available, with the vascular surgeons leading the way nationally and operating on many
symptomatic patients within 2 weeks. This has been developed over the last 2 years as a
trust project. CT with e-ASPECTS technology, CTA and CTP are available 24/7.

Southend Stroke service is award-winning and innovative. It has won 2 HSJ awards for the
quality of its care, has been involved in the development of e-ASPECTS which won a
European award for innovation. We are very involved in NIHR research, being invited to take
part in commercial and NIHR portfolio trials, and last year recruited 24.3% of patients into
research trials, the second highest in North Thames CRN. International links with the USA
and Germany have enabled us to be one of only 4 hospitals in the UK able to perform
functional resting state MRI.

Recently there has been a complete revision of the imaging services, so that now there is a
Consultant-led service with the ability to initiate correct scanning; instant access to
CT/CTP/CTA for urgent cases; and pre-booked slots for routine CT, MRI imaging and
clinician led carotid Doppler ultrasound.

Research and Audit activity is encouraged and we have strong academic links with the Anglia
Ruskin University. There is a very active Stroke research unit with reputation of among the
top recruiter in many trials. As the Trust has a single independent specialist hyper acute
Stroke Unit in one geographical location and a specialist stroke team this has led to a 50%
reduction in mortality and dependency; and almost all stroke inpatients spend all their
hospital stay on the Stroke Unit. All NICE guidelines have been incorporated into protocols.
There are monthly governance meetings to continually improve care.

Aims of Post

• Develop enhanced clinical skills, especially in the assessment and management of


acutely ill patients in A&E

• Follow patients throughout their clinical pathways

• Gain experience in general practice, and the interface between primary and secondary
care

• Improve all aspects of communication with patients, carers and staff members working
within multi-disciplinary teams

• Develop knowledge of hospital systems and clinical governance

• improve competencies in IT skills and critical use of evidence and data

• keep accurate records and completing discharge summaries

MAIN DUTIES & RESPONSIBILITIES

• Liaise between nurses, other junior doctors, patients, relatives and senior medical staff

• Attend to the day to day management of day case, out-patient and in-patients

• Attend and participate in ward rounds

• Attend general and specialist out-patient, diagnostic and pre-assessment clinics

• Teach medical students as directed

• Supervise, monitor and assist the F1 doctors.

• To keep up-to date with all administrative duties including letters, results and discharge
summaries as required

• Maintain continued professional development

• Undertake and assist in audit projects

• Attend weekly educational and multidisciplinary sessions

• To co-operate with members of the HR department when monitoring hours of work

• Comply with all local policies including dress code, annual and study leave

This post has neither Royal College nor Postgraduate approval for training.

The post is, however, educationally sound and well supported. The Post holder will be
allocated an educational mentor.

Ward Based Working

The post holder will contribute to the care of patients on a specified medical ward. The
patient mix of each ward is biased towards the speciality of the supervising consultants, but
also includes general medical patients with non-speciality specific conditions.

Clinical Governance

To provide clinical services in line with the Trust’s clinical governance arrangements
which is designed to ensure that agreed quality standards are achieved. These
requirements include:-

• AUDIT – To undertake audit of clinical practice within the department to ensure that
current standards and evidence-based practice are applied.

• CONTINUING MEDICAL EDUCATION – All staff are required to maintain and develop
their clinical skills. Individual training and development needs will be identified through
an appraisal process taking into account Mid and South Essex NHS Foundation Trust
requirements.

• RISK MANAGEMENT – To work within the Trust’s clinical risk management policies
and in particular to participate as appropriate in clinical incident reporting.

Teaching and Training Activities

The post-holder will be expected to provide supervision and support for more junior trainees
within his/her competence. Although the post does not have postgraduate approval for
training, assessment of progression will be by continual assessment.

Clinical Governance is a major part of our daily practice. The hospital is at the forefront of
providing same, with a separate department and allocated clinical and ancillary staff to service
same.

There is a well-stocked medical library with audio-visual teaching aids, CD-ROM and a full
time librarian. The library has a Medline facility. There is a thriving Postgraduate Medical
Centre with regular clinical meetings and a general education programme.

Main Terms and Conditions of Service

The appointment is subject to the 2002 Medical and Dental Terms and Conditions of Service.
The Clinical Fellow FY2 level pay scale starts on £31,001 per annum.

Banding

You will receive, in addition to your base salary a supplement at a rate of 50% (1A) of your
base salary for full shift duty including late, night and weekend shifts.

GENERAL STATEMENTS

COMPETENCY BASED APPRAISAL

The post holder will be appraised on an annual basis and the process will include a review
of the past year’s performance, setting of aims and objectives for the coming year and
identification of educational needs.

TRAINING

All staff will be required to keep mandatory training records up to date. All Medical staff are
required to participate in CPD as part of the Appraisal and Revalidation process.

INFORMATION GOVERNANCE

• All staff must be familiar with and comply with the contents of the Information Governance
Handbook, a personal copy will be provided at Induction to all staff.
• All staff are required to maintain confidentiality of patient and Trust’s information as set
out in the Trust’s Confidentiality Policy.
• All staff are required to read and comply with all policies that are issued relating to the
electronic security of Trust’s information.

• All staff who create, access, transfer, modify sensitive Trust’s records have a
responsibility to be both accurate and timely and ensure that all the information that they
record either on paper or electronically is complete.

CONFIDENTIALITY

Your attention is drawn to the confidential nature of information collected and used

information is a dismissible offence. The unauthorised disclosure of information could also
result in a prosecution for an offence, or action for civil damaged, under the Data Protection

competency level appropriate to the work you do and in accordance with the Trust’s

LOCATION

You will be based at Care Group 3 which is located in Southend. You will generally be
expected to undertake your normal duties at the principal place of work, however, in the
course of your duties you may also be required to work at any of the sites within the Mid and
South Essex NHS Foundation Trust areas as well as other NHS sites as part of the delivery
of the service.

Other Information

This job description is a reflection of the present requirements of the post and the content
may be subject to review and amendment in light of changes and developments.

to their discipline.

Terms and Conditions of Service

The post is subject to the Terms and Conditions of Service of Hospital Medical and Dental Staff
(England and Wales) and to the NHS Pension Regulations. You will receive the nationally agreed
remuneration for Trust Foundation Doctors in Hospital employment and any changes to those rates
that the Secretary of State for Health may authorise from time to time.

The salary scale depending on experience for full-time employees will be as below (National Medical
and Dental Pay Circular):

Hours: 40 hours per week

A discretionary banding supplement determined by of the Terms and Conditions of Service maybe
applicable for hours worked over and above 40 hours a week to a maximum of 48 hours including
unsocial hours depending on the rota.

Part time staff will be paid pro rata to this scale.

The appointment can be terminated by giving the required notice on each side, unless application of
the statutory minimum period would be more favourable to the Practitioner.

FY1/2 1 months’ notice

Relocation Expenses:

Relocation expenses may be granted on the understanding that the new location meets the
requirements of the Trust. Agreement regarding relocation expenses must be approved by the
service.

Leave

Annual leave entitlement is set out within the Terms and Conditions of Service for hospital medical
staff.

Study leave is available as provided for under the Terms and Conditions of Service for Hospital
Medical and Dental staff, subject to the approval of the Supervising Consultant and Clinical Director.
The General Manager has final overall approval of all leave (study leave, annual leave,
compassionate leave and professional leave).

Appraisal and Development

This is conducted by a trained, Trust-nominated appraiser. It is expected that the successful


candidate will comply with the appraisal process. Yearly appraisals are a mandatory requirement as
a part of the GMC Revalidation cycle for all medical staff in the NHS.
