MINISTER OF JUSTICE OF THE REPUBLIC OF INDONESIA

Reading : 1. The application letter dated June 27, 1989

From “ LIE, WEI TJING”
Residing at Jl. Ciliwung No. 44, Semarang
To give him the permission to change his name and hereinafter to use the
family name of “LUKITO” with the given name of “Indrawan Tjahjono”

2. The statement letters on the approval of the name change from:

a. The Mayor of Semarang City
Dated May 18, 1989 No. 22/GN/1989;
b. The Head of the Semarang City Police
Dated April 26, 1989 No. Pol. B/SK/404/IV/1989/INTELPAM

Considering : a. That the intention to change the name as mentioned above had been declared
in the State Gazette of the Republic of Indonesia dated July 21, 1989 No. 58.
b. That noone states his/ her objection on the name change in a particular
period of time

In view of : 1. Law No. 4 of 1961 (the State Gazette of the Republic of Indonesia of 1961
Number 15, the Addendum of the State Gazette Number 2154).
2. the Presidential Decree No. 32 of 1988 on the Organizational Structure of
the Ministry;
 DECIDE

Determining :
FIRST : Granting the permission to:
“LIEM, ING TIONG”, born in Semarang dated January 4, 1950 (the deed No.
21/ 1950).
To change the family name of “LIEM” into “LUKITO” and the given name of
“Ing Tiong” into “Indrawan Tjahjono” so that hereinafter she has the name of:

“Indrawan Tjahjono LUKITO”

SECOND In order that the decree is announced in the State Gazette of the Republic of
: Indonesia and registered in the local civil registration office to be recorded
properly.
THIRD This decree is delivered to the person concerned to be used properly with the
: note that there will be revisions in the future when the mistakes are found in this
decree.
