Dear Sir,
I/We _________am the Director/Partner/Proprietor/Authorized Representative of ____________ an Online
Examination Venue located at ____________(Address)________________________________,do hereby give the
consent to conduct the ________(Examination Name)_____________________________________ at our Venue
from _______to _______ . I/We are also providing the venue ______ days prior to the examination for mock and
readiness. I/We do hereby declare and undertake as under:
1. This is to confirm that I/We (Name: ______________________________________________, Aadhar No.
______________________) the Director/Partner/Proprietor/Authorized Representative of _____________
and my exam center staff have been / will be conducting exams in a fair and transparent manner and
will continue to do so.
2. I/We declare that I/We are of good standing, and I/We are not barred by any Court of India/any Central
Government / State Government / Statutory Authority from entering into any Work Order with APTECH
as on date of giving this declaration.
3. I/We undertake that it is our sole responsibility to successfully conduct the examination and in case of
any failure, be it technical fault/glitch, software breach, security breach, cheating or malpractices used
before or during the exam, any type of breach or failure is directly and solely attributable to us and
APTECH will in no way be liable and answerable for the same.
4. I/we declare that I/we have carefully gone through the terms and conditions of this Work Order and I/we
shall fully comply and abide by the same.
5. I/We hereby certify that our Institute is a reputed online examination venue and has been conducting
various Central/State Government examinations successfully since ____________ and we undertake to
provide the appropriate documents for the same.
6. I/We undertake to provide required number of systems (<Booked Node Capacity>) with proper LAN
connectivity with updated security measures, firewalls and such other anti-virus (all type of hacking)
with no malwares and desired system configuration for conduct of online examination.
7. I/we undertake to provide sufficient, ethical and non-corrupt manpower with no criminal background or
antecedents for the smooth conduct of the examination.
8. We will provide adequate and experienced manpower as invigilators with experience in online exam
conduction as per the ratio detailed above.
9. I/We agree that we will not compromise the confidentiality and security of the Examination.
10. No other exam/activity would be scheduled at the test center/labs assigned for above mentioned exam
on the scheduled date.
11. I/we declare that none of our staff member or any of their relative is appearing in this examination.
 12. I/We declare that that I, or any of the staff members of ________ are not involved in any
investigation/ongoing investigation with the SIT/CBI/POLICE related to any incident of exam
malpractice / misconduct.
13. I/We declare that I/we have not been blacklisted by any of the Central/State Governmental bodies,
Quasi-Judicial Bodies or by any decree/order/judgment passed by any Courts of India.
14. I/we declare that our services/facilities/Agreement have not been terminated in the past by any of the
Central/State Governmental bodies, Quasi-Judicial Bodies or by any decree/order/judgment passed by
the Court, any private entity.
15. I/We declare that there is no dispute either civil or criminal pending with the Central/State
Government/Statutory Authority/Private entities like Private corporations/Companies/ Partnership
Firms/Previously entered Joint Ventures as on date of giving this declaration.
16. I/we undertake that in case of any criminal investigation/complaint instituted by any Criminal
Investigating Agency, Center shall notify and intimate APTECH within 24 (Twenty-Four) Hours of
receiving the notice/summons/call and center shall not give any reply to the investigating Agency
without the approval of APTECH.

All the staff are healthy and does not have any history of COVID 19 for the past 3 months. None of the test
center staff is coming from Containment Zone. Also, complete hygiene will be ensured during the exam.
