I, Kimberly T. Salvador, do hereby authorize Jeaneth B. Agcaoili, to get our refund from
your good office. She is my fellow teacher in Maligaya Elementary School-Main. I am
currently not able to personally do the transaction because I have an important
appointment to attend.

I am also authorizing Jeaneth B. Agcaoili to sign and process any required steps to
complete the transaction. She will be representing her Identification card or passport for
verification. I understand that my representative will not be responsible in cases like loss
of documents or delays.

Jeaneth B. Agcaoili is exclusively allowed to receive the amount and to use it for any
other purposes. This letter is effective from May 25 until May 28 only.
Attach here is my two-identification cards.
