Good day everyone, I’m Suerte Agustin and I am here to demonstrate and perform on how to assess

the Musculoskeletal system.

We need to assemble all equipment and supplies, the PPE we have a PPE gown, face mask, face shield,
examination gloves, hair cover, and also the goniometer. All equipment required for the health
assessment should be clean, in good working order, and readily accessible.

We need to explain the Procedure to the client. Discuss how the result will be used for further care or
treatments.

Hi my name is Suerte Agustin and I am your nurse student for today, and I will be doing assessing your
Musculoskeletal.

I already wash my hands to prevent the spread of contamination and to observe appropriate infection
control procedures.

It is important to provide for client privacy.

So, sir I’m going to close the door and the curtains for your privacy, is that okay with you po?

First, I’m going to ask you some questions, is that okay po?

What is your name?

When is your birthdate?

What would you like to call you?

To determine the client’s history of the following:

Do you any history or presence of muscle pain: onset, location, character, associated phenomena, and
aggravating and alleviating factors?

Any limitations to movement or inability to perform activities of daily living?

Do you have any previous sports injuries?

Any loss of function without pain?

~okay, that’s good to hear sir.

And now were going to start the procedure.

First, inspect the muscles for size, for the muscles and tendons for contractures, and also for muscles
for fasciculations and tremors.

Compare each muscle on one side of the body to the same muscle on the other side. For any apparent
discrepancies, measure the muscles with a tape.

~As I inspect, equal size on both sides of body, No contractures, and No tremors.

Palpate muscles at rest to determine muscle tonicity.

~ The muscle tonicity is normally firm.

Palpate muscles while the client is active and passive for flaccidity, spasticity, and smoothness of
movement.

~The movement of the muscles are smooth and we are doing this to know the Smooth coordinated
movement Deviations from normal Flaccidity (weakness of laxness) or spasticity (sudden involuntary
muscle contraction).

Test muscle strength. When assessing the muscle tone and strength, it is necessary to compare one
side with the other side.

~ Equal strength on each body side.

A. Sternocleidomastoid – Place hand on the side of the patient’s head. “Turn your head toward my
hand.”
