DEED OF DONATION

KNOW ALL MEN BY THESE PRESENTS

This Deed of Donation is made and executed this _____ day of ________________ at
_______________________, by:

(NAME OF DONOR), of legal age, Filipino, single/married/widow and

with residence address at _____________________ (hereinafter referred to as the
“DONOR”);

- in favor of -

(NAME OF DONEE), of legal age, Filipino, single/married/widow and

with residence at ____________________________ (hereinafter referred to as the
“DONEE”);

WITNESSETH; That:

Section 1. The DONOR is the registered owner of a parcel of land, together with the
improvements thereon, located at ______________________. The said property is more
particularly described as follows(the “Subject Property”):

Section 2. For and in consideration of the love and affection which the DONOR has for her
child, the DONEE herein, the DONOR by these presents transfers and conveys to the DONEE, his
heirs, assigns and successors-in-interest the Subject Property.

Section 3. The DONOR does hereby state, for the purpose of giving full effect to this
donation, that she has reserved for herself in full ownership sufficient property to support her in a
manner appropriate to her needs.

Section 4. The DONOR hereby reserves for herself, as long as she lives, the right to all the
fruits and rents that may be derived from the Subject Property donated for her exclusive use, benefit
and disposition, and meanwhile the DONEE may not sell, alienate, exchange, mortgage, or in any
manner encumber the Subject Property without the prior written consent of the DONOR.

Section 5. The DONEE do hereby accept the donation of the above-described property, and
do hereby express their gratitude for the kindness and liberality of the DONOR.

Section 6. This Deed may be executed in two (2) or more counterparts and by the parties hereto on
separate counterparts, each of which when so executed and delivered shall be an original, but all of which
shall together constitute one and the same instrument.

The instrument referred to is a Deed of Donation consisting of three (3) pages including
the page on which this Acknowledgement is written duly signed by the parties and their
instrumental witnesses on each and every page hereof.
