Ordering Physician: Dr. Castro

Brief description: A blood transfusion is a routine medical procedure in which donated blood is
provided through a narrow tube placed within a vein in the arm.

Purpose: This was given to our patient to raise the level of Hgb and Hct within the normal
range.

Nursing Responsibilities Rationale

1.     Secure the consent for Blood  To provide a structure for a patient to make
transfusion an informed choice regarding the
indications, risks, possible alternatives, and
benefits of a blood transfusion.

2.     Change the present IVF of D5LR 1L to  To match the concentration of the blood
