What is a HOME VISIT?

● Family-nurse contact
● Opportunity for the health worker to assess the home and family
● To provide necessary nursing care
● A plan of visit is essential to achieve desired outcomes

PURPOSE OF HOME VISIT

1. To give nursing care to:


• the sick
• A post partum mother and her newborn
- To teach a responsible family member to give the subsequent care

2. To assess the patient and his family


• Living condition
• Health practices
- To provide appropriate health teaching

3. To give health teachings:


• Prevention and control of diseases

4. To establish close relationship between health agencies and the public


• For the promotion of health

5. To make use of the inter-referral system and to promote the utilization of


community services

PRINCIPLES OF HOME VISIT

1. A home visit must have a purpose or objective.


2. Planning for a home visit should make use of all available information… through
family records.
3. Consider and give priority to the essential needs of the individual and his family.
4. Planning and delivery of care should involve the individual and family
5. The plan should be flexible
CONSIDERATIONS ON FREQUENCY OF VISIT

1. Needs of the individual and family


2. Acceptance of the family, and their interest and willingness to cooperate
3. Policy of a specific agency
4. Other agencies and number of health personnel involved in the care of the family
5. Evaluation of past services and how the family availed them
6. Ability of the client to recognize own needs and resources

STEPS IN CONDUCTING HOME VISITS

1. Greet the patient and introduce yourself.


2. State the purpose of the visit.
3. Perform assessment.
4. Perform BAG TECHNIQUE.
5. Perform nursing care and give health teachings.
6. Record all important data.
7. Make appointment for a return visit
