PROMISSORY NOTE

1. THE PARTIES. This Promissory Note made on April 28, 2023, is by and between:

LESSOR: Kamille Patricia C. Carlos with a mailing address of Red Latan St.,
Palma De Oro, Dolores, City of San Fernando, Pampanga, and

LESSEE: Ericka Omo with a mailing address of [MAILING ADDRESS]

(“Lender”).

2. UNPAID DUES. The Lessee has an outstanding balance for 5 MONTHS for a total
of SEVENTY FIVE THOUSAND PESOS ONLY (Php75,000) Philippine Currency.
And unpaid bills for Electricity for a total of FIFTY SIX THOUSAND THREE
HUNDRED EIGHTY FIVE PESOS ONLY (Php56,385) Philippine Currency
 
3. PAYMENTS. The full balance of the UNPAID DUES, is due and payable in;

☐ - INSTALLMENTS. Lessee shall pay installment amounts equal to ONE

HUNDRED THIRTY ONE THOUSAND EIGHT HUNDRED THIRTY FIVE PESOS
ONLY (Php131,835) Philippine Currency with the first (1st) payment due on
[DATE] and the remaining payments to be paid: (check one)
