1. Open Get Blank Form

Tap Get Blank Form in SurveyCTO

Collect to open that menu.
You must get blank forms before you can
fill them out.

2. Confirm credentials

If you see this popup, tap OK.

If the popup appears after tapping OK,
go back to the main menu, open the
three-dot menu, return to the General
Settings, and confirm that your
username and password are correct.

3. Select form(s)

Tap on the form(s) you would like to

download.
If you do not see the form you want, tap
Refresh. Only deployed forms will
appear (not drafts).
You can also select all forms by tapping
Toggle All at the bottom.

4. Download form(s)

Tap Get Selected to download the

form(s) you have selected.
Quick start: Get form 6 of 8

5. Confirm download

At the popup, tap OK.

To return to the main menu, tap the
device's back button.

Tip: Using web forms

Deployed forms can also be filled out as web forms in a browser.
