I hereby certify that Mr/Ms Ramadhani Msangi with passport
no. TAE181797 has been tested for COVID-19 using PCR
method on 2021-01-16 at National Public Health Laboratory
and found to be NEGATIVE. Therefore, the afore-mentioned
is allowed to proceed with his/ her daily duties. This
certificate is valid for 14 days after testing however it does
not guarantee future infection. The bearer should observe
infection prevention and control measures including regular
hand washing with soap, use of mask and observing physical
distancing as applicable.
