REQUIREMENTS

Applicants must present evidence of verifiable CDEs (CCERPS, CPD, etc.) over a five-year period,
on implant-related material, totaling 200 hours in 18 specific categories. 85 of those 200 continuing
education hours are considered “core knowledge”.
Applicants will also need to provide verifiable evidence of continuing education hours from certified
providers. Evidence may include Letter of Verification, certificates, etc. Documents can be sent digitally
either online through www.dropbox.com (or another comparable file sharing site) or compressed in a
zip file not to exceed 5 MB.
Applicants must present evidence of verifiable CDEs (CCERPS, CPD, etc.) over a five-year period, on
implant-related material, totaling 200 hours in 18 specific categories. 85 of those 200 continuing
education hours are considered “core knowledge”.
The following exceptions apply:
• Members who possess a mono-specialty certificate, AEGD certificate or GPR certificate older than
5 years will have the 85 hours of required core knowledge credits waived. If you have achieved this
certificate more than five years ago, please complete ONLY Part I of the Continuing Education and
Core Knowledge Requirements application section and provide a copy of your certificate when you
submit your application. You may skip Part II.
• Members who have a mono-specialty certificate, AEGD certificate or GPR certificate within the last
5 years will have the 200 CE hours requirement waived. If you have achieved this within the past
five years, you may skip this form and instead provide a copy of your certificate when you submit
your application.
