APPLICATION FOR PROBATION

COMES NOW, the accused, BOY ADAN, by himself unto this Honorable Court,
most respectfully avers:

1. That on arraignment on October 17, 2013, accused pleaded GUILTY to the crime
of Adultery and was meted an indeterminate penalty of four months and one day of
arresto mayor, as minimum, to two years, four months and one day of prision correccional, as
maximum, of imprisonment.

2. That judgment against the accused was promulgated on October 17, 2013.

3. That accused have not been previously convicted by final judgment of any crime
or offense punishable by imprisonment of not less than one (1) month and/or a fine of
not less than Two Hundred (Php.200.00) Pesos.

4. That accused have not previously availed nor applied for probation under the
provisions of P.D. 968, as amended.

5. That the accused are qualified and does not possess any of the disqualifications
for probation under the afore-said law.

6. That the accused desires to avail of the Probation Law and promises to faithfully
obey any and all conditions imposed upon them.

PRAYER
 WHEREFORE, premises considered, it is most humbly and respectfully prayed
that this Honorable Court issue an order granting this application for probation.
Other just and equitable relief is likewise prayed for.
