Purpose
The purpose of this procedure is to guide towards providing a safe execution during
tank refurbishment works at all Benua Muhibah Project sites

Scope
This procedure applies to all Benua Muhibah Ent personnel, Subcontractors and their
employees. It shall apply to activities at the site for various projects.

General Procedures
The general procedure for Refurb Diesel Storage Tank will vary depending on whether it is
deemed to be accessible and safe. The following are standard basic procedures that should be
followed for Customer Diesel Storage Tanks that are accessible and safe for Cleaning and
Refurb.

Requirements for HSE


 The Culpable Person shall keep Diesel Storage Tanks in a tidy, clean and safe condition
and shall make sure that Diesel stored within such Tanks is safe.
 The accountable Person shall ensure a safe working environment in and around
Customer Diesel Storage Tanks.
 The Culpable Person shall remove or control any environment, health, safety &
Environmental (HSE) hazards on their Premises to ensure safe access or Inspection,
Sampling, Cleaning and Disinfection of Customer Diesel Storage Tanks by Responsible
Companies.
 The accountable Person shall ensure any HSE risk control measures identified by a
Responsible Organization are put in place on time to enable the Inspection, Sampling,
Cleaning or Disinfection of a Diesel Storage Tank.

Working in Confined Space


Before entry (If Entry into tank required), the permit should be processed by a “qualified
&competent person” and signed by the entrant (work performer) and supervisor/project
manager. If an emergency exists, a “qualified/competent personal” can sign the permit. The
completed permit shall be made available to all entrants; occupants and attendants by
posting near the entrance of the confined space. The supervisor or “qualified person” has the
authority to terminate the permit if conditions affecting the entry exceed the scope of the
permit.

Observe activities internal and external to the confined space to regulate if it is safe for
entrants to remain in the space and order the authorized entrants to evacuate the confined
space as soon as possible under any of the below-mentioned conditions:

1. If the attendant reveals a restricted condition;


2. If the custodian observes the behavioral effects of hazard exposure in an authorized
entrant;
3. If the attendant observes a situation external to the space that could imperil the
authorized entrants; Cannot effectively and safely perform all the required duties;
Carry out the following strict actions when unauthorized personals approach or enter a
permit space while entry is ongoing:
4. Summon the unapproved personals that they must stay away from the confined space
permit;
5. Instruct the unofficial personals that they must exit immediately if they have entered
the permit space;
6. Inform the unofficial entrants and the entry Forman if unauthorized persons have
entered the confined space;
7. Perform non-entry rescue only and does not attempt to rescue entrants by entering
the confined space.
8. Execute no duties that might interfere with the attendant's initial duty to monitor,
observe and protect the authorized entrants.

Hot work general good practices


Make sure you are following your hot work procedure. Also consider the following items:

 Make sure that all equipment is in good operating order before work starts.
 Make sure that all appropriate personal protective devices are available at the site and
each worker has been trained on how to use, clean, and store them properly.
 Inspect the work area thoroughly before starting. Look for combustible materials in
structures (partitions, walls, ceilings).
 Move all flammable and combustible materials away from the work area.
 If combustibles cannot be moved, cover them with fire resistant blankets or shields.
Protect gas lines and equipment from falling sparks, hot materials, and objects.
 Sweep clean any combustible materials on floors around the work zone. Combustible
floors must be kept wet with water or covered with fire resistant blankets or damp
sand.
 Use water ONLY if electrical circuits have been de-energized to prevent electrical shock.
 Remove any spilled grease, oil, or other combustible liquid.

 Vacuum away combustible debris from inside ventilation or other service duct openings to
prevent ignition. Seal any cracks in ducts. Prevent sparks from entering into the duct work.
Cover duct openings with a fire resistant barrier and inspect the ducts after work has concluded.
 Make sure that appropriate fire extinguishers (e.g., ABC fire extinguishers) are available and
easily accessible.
 Make sure that the first-aid boxes are available and easily accessible.
 Block off cracks between floorboards, along baseboards and walls, and under door openings,
with a fire resistant material. Close doors and windows.
 Cover wall or ceiling surfaces with a fire resistant and heat insulating material to prevent ignition
and accumulation of heat.
 Secure, isolate, and vent pressurized vessels, piping and equipment as needed before beginning
hot work.
 Inspect the area following work to ensure that wall surfaces, studs, wires or dirt have not heated
up.
 Post a trained fire watcher within the work area, including lower levels if sparks or slag may fall
during welding, including during breaks, and for at least 60 minutes after work has stopped.
Depending on the work done, the area may need to be monitored for longer (up to 3 or more
hours) after the end of the hot work until fire hazards no longer exist.
 Eliminate explosive atmospheres (e.g., vapors or combustible dust) or do not allow hot work.
Shut down any process that produces combustible atmospheres, and continuously monitor the
area for accumulation of combustible gases before, during, and after hot work.
 If possible, schedule hot work during shutdown periods.
 Comply with the required legislation and standards applicable to your workplace.

Tank Refurb Procedures


Pre-Checks
o Select the workers and screen for competency and Physical fitness to perform the
activity.
o Ensure HSE induction, First Aid, working in Confined space training are provided
before engaging them to work
o Ensure all workers at the site wear safety shoe, helmet, full-body harness and gloves
during tank cleaning works.
o Ensure Fully Equipped First Aid Box should be made available at each location.
o Ensure Emergency contact details are listed.
o Ensure the Confine Space permit is implemented before starting tank cleaning
works.
o Ensure all tools and equipment for the cleaning work are well maintained. Do not use
units/parts that are in poor condition.

o Ensure & inspect the tank before entering for its safety. Follow Confined space permit
procedures.
o Ensure weather condition is normal before starting to climb. Do not Work under
abnormal weather conditions i.e., storm, wet conditions or high winds etc.…

Diesel Tank Storage Refurbishment Guidelines


Secure access and required authorizations to enter the building and conduct the Tank
refurbishment.
 Notify tenants of the proposed Tank Cleaning date and time at least 48 hours before
Cleaning.
 Shut off inlet valve at least 24 hours before Cleaning to allow maximum Diesel to be
used (to ensure less wastage of Diesel).
 Prepare all necessary Tank refurb equipment, and personal protective equipment
(PPE) off-site and before Cleaning.
 Arrive on-site and notify owner through notices of the task and expected duration and
precautions they should take.
 Place safety barriers around equipment and working area, if required.
 Tank inlet valve and all other outlets, including instrument tubing should be closed
previously to allow the Tank to empty.
 Lock-out/tag-out all Tank inlets and outlets and make sure signs are placed notifying
that they should not be opened.
 Proper ‘lock-out tag-out procedure and signage
 To disconnect all piping and fittings to the Tank.
 To safely drain out remnant diesel to through or sump as directed by owner
 Ensure the draining activity is always connected to earthing to avoid statis electric
spark or flash,
 Open the Tank cover and allow the Tank to vent for 20 minutes.
 Determine safe oxygen and gas levels using a gas detector. If safe, insert air exchange/
exhaust fan hose.
 Measuring safe gas levels before opening Diesel Tank cover.
 Drain the Diesel remaining in the Tank using either the outlet valve (if available) or
sump pumps (if the outlet valve is not available).
 Different draining arrangements
 Set up equipment, including any fall protection and confined spaces equipment and
PPE.
 Proper confined space and fall protection procedure and equipment
 Cleaning Non-Man-Entry
