I: IDENTIFYING DATA
Client’s Name: EF
Sex: Female
The client sought solution for her workers requested for transfer due to increased
demands on work.
During her first weeks on the job, Miss EF made no changes in establishing
procedures, but concentrated on getting acquainted with staff, observing them at work;
and sometimes helping out in easing the heavy caseloads of some workers.
She found out that there are backlogs continue in their office and rather than tell
them of her opinion, however she decided to require each worker to submit daily
activities report instead of the usual monthly report. The report was supposed to include
the number of clients interviewed, problems presented by the clients, action taken by
the worker as well as time spent for each client and it ended up to the social worker to
request a transfer to another department because of the pressure they take.
Miss EF sought solution on how to pull back all backlogs taken by their office.
Miss EF showed great motivation and interest to participate and cooperate for the
problem in their office.
The potential solution for Miss EF’s problem is quite easy because the resources
and services that could help her are just available in their office. Since she is a
Registered Social Work and entitled of Master of Science in Social Work, she has a
wide range of understanding on how to solve different problem by applying principles of
Social Work and it will be a big help for her.
If Miss EF’s problem will not addressed immediately, it will cause her emotional
stress, and will also affect her personal life because of work. On the other hand, if her
problem will immediately cater, it will be a big help for her and for her department.
During adulthood, we continue to build our lives, focusing on our career and family.
Those who are successful during this phase will feel that they are contributing to the
world by being active in their home and community. Those who fail to attain this skill will
feel unproductive and uninvolved in the world.
V: INTERVENTION PLAN

VI: EVALUATION
With the client's participation, all information necessary for the helping process
has been gathered through intake interviews during the initial phase up to the helping
process.
VII: RECOMMENDATION
The study shows that the client needs and other possible services that could help
her co-workers to think has been catered: therefore, the client’s worker is recommended
to have weekly assembly to know further problem if there’s.
Miss EF was also recommended to monitor the condition of her co-workers in
their department.
She also recommended to take care of her self so that she could bring more
positive outcome in their department office.
