We invite you to participate in a research study entitled “ROLE OF COOPERATIVES

IN IRIGA CITY FOR SUSTAINING LIVELIHOODS OF ITS MEMBERS”. The purpose of this
research is to measure perception of respondents, their attitude, behavior, and
rationalization regarding cooperatives in sustaining the livelihood of the members.

Answering this survey will take approximately 5 minutes of your time. If you agree to
participate in this research, please answer the questions as honestly and best as you can.
We will not collect your email addresses/personal information without your permission. We
are committed to comply with the Philippine Data Privacy Act of 2012 or RA 10173 in
handling your data with confidentiality and anonymity. Data will only be presented as a
