Greetings of peace and prosperity!

I, Mary Grace D. Sendrejas, permanent teacher 1 at Cervantina Elementary School, Carmen 2 -District,
Carmen Agusan del Norte for two (2) years and seven (7) months. I am a bona fide resident of District 11, Macopa,
Poblacion, Carmen, Agusan del Norte.

I am requesting a transfer to any of these schools closer to my family which will enable me to better serve the
community's needs.

1. Carmen Central Elementary School

2. Quiamjot Elementary School
3. Elpedio Salas Sr. Elementary School

These are my reasons for transfer:

1. Family reasons: I need to take care of my 3-month-old infant, my 2-year-old toddler, and my 6-year-old son.
2. Health reasons: My 3-month-old baby has DANDY-WALKER MALFORMATION SECONDARY TO
HYDROCEPHALUS. My child badly needed maternal care due to his hydrocephalus. If I would be given a
chance to transfer immediately. It really helps me caring and monitoring my child from time to time.
3. Work-life balance: As a mother of three (3), I may find it challenging to balance my job as a teacher and my
responsibilities as a mother. A transfer to a school with more flexible hours may be necessary to achieve a
better work-life balance.

It is important to note that everyone’s reasons for transferring can be unique and personal, and it is essential to
understand and address these reasons to ensure a smooth transition.

I am aware that the transfer process may take some time, and I am willing to provide any necessary
