DOD -5.5mm. Normal Bolton index. Normal transverse skeletal discrepancy.

Family history: class III mandibular (mother), primary crowding (Father and Mother and a brother),

and chronic adult periodontal disease (Father)

Personal history: Oral breathing. Adenoidectomy at 7 years. No previous orthodontic treatment

has been performed. Proper oral hygiene. Rest without interest.

Exploration: Mixed breathing with low posterior position of the tongue at rest. Thin periodontal

biotype. Lip incompetence with habit of mind.

Palpation: All normal. Mandibular Dynamics: Symmetrical mastication. rest without interest

Diagnostic Summary of Cephalometry:

1. Moderate-Severe Dolichofacial Pattern (2-3DS). Growth with posterior rotation and center of

rotation in the Condyle. Good dentoalveolar adaptation. Occlusal plane is maintained within the

first SD despite much greater posterior rotation of the mandibular plane.

2. Moderate skeletal class II (just 2 SD), due to mandibular posterorotation (SL of 44mm)

3. Severe protrusion (3DS) and moderate proclination (2DS) of the lower incisor

Normal upper incisor (slight proclination??. Limit 1DS)

Perform the Steiner box and determine if therapeutic extractions are necessary or not, and the
needs for anchorage in the two arches
