PRIMARY HEALTH CENTRE REPORT
INTRODUCTION: PHC is the 1st contact point between village community & the medical officer. The
PHC were envisaged to provide an integrated preventive health care to the rural and urban population
with emphasis on prevention of provocative aspects of health care. The PHCs were established &
maintained by the state government under the minimum needs programme / basic minimum services
programme.

FUNCTIONS OF PHC:
The functions of primary health centre in India cover all 12 essential elements of primary health care.
These are:
❖ Medical care referral services for subcentre.
❖ Maternal & child health including family planning.
❖ Safe water supply & basic sanitation.
❖ Prevention of control of locally endemic diseases.
❖ Collection & reporting of vital statistics.
❖ Education about health.
❖ National health programme
❖ Referral Services
❖ Training of health workers, local dias & health assistants.
❖ Basic laboratory services
❖ Tubectomy / Vasectomy Services
❖ Health & Nutrition Education

OBJECTIVES OF PHC ACC TO IPHS:


❖ To promote health & efficiency of given population.
❖ To prevent and control diseases and disabilities.
❖ To prolong life by providing need based, well balanced, comprehensive healthcare services to
community at large level through organised community efforts.
❖ To prevent and control communicable and non-communicable diseases.
❖ To conduct research.
❖ To give health education to community.

POPULATION UNDER PHC & HWCs :-


PHC: It is established to cover about 30,000 population in plain and 20,000 population in hilly areas.
There are 4 - 6 beds with one medical officer.
HWC: Health and Wellness Centre can provide basic medical services to a cluster of population of 5000
- 15,000 people in rural, suburban and urban setup.
Our PHC in which we are posted situated in KOHKA :-
➢ AREA: - 3.94km2
➢ POPULATION (2020): - 17203
➢ MALE POPULATION: - 8803
➢ FEMALE POPULATION: - 8400
➢ NEAREST FRU CENTRE:- Lal Bahadur Shashtri Hospital Durg, District Hospital Durg
➢ TIMING: - 10 AM to 5 PM
➢ OPD REGISTRATION: - 4:30 PM

HEALTH & WELLNESS CENTERS:


Ayushman Bharat Health and Wellness Centres (AB – HWCs) were launched under the Ayushman
Bharat Programme in a bid to move away from selective healthcare to a more comprehensive range of
services spanning preventive promotive, rehabilitative and palliative care for all ages.
Health centres would deliver Comprehensive Primary Health Care (CPHC), being
healthcare clones to homes of people covering both maternal and child health services. Non
communicable diseases including free essential diagnostic services.

EXPANDED RANGE OF SERVICES:


Expansion of services has been planned in inuremental manners ie. first step. Screening, prevention,
control and management of non-communicable disease and chronic communicable diseases like TB
and leprosy has been introduced to HWCs
❖ Care in pregnancy & child birth
❖ Neonatal, infant health care services
❖ Childhood and Adolescent healthcare services.
❖ Family planning, contraceptives services and other reproductive health services.
❖ Management of communicable diseases, National Health programmes.
❖ Management of Common Communicable Diseases at General output care for acute simple
illness and minor ailments.
❖ Basic oral care.
❖ Care of common ophthalmic and ENT problems.
❖ Emergency medical services.
❖ Elderly and palliative healthcare services.
❖ Screening and basic management of mental health ailment.
KEY PRINCIPLES OF HWCs :-
➢ Transform existing sub centres and health centres to Health and Wellness centres to ensure
universal access to an expanded range of comprehensive primary healthcare services.
➢ Ensure a proper centered, holistic, equity sensitive response to people’s health needs through a
process of population empandment, regular home and community interactions and peoples
participation.

IMPORTANCE OF WELLNESS CENTRE :-


The main purpose of Wellness centre is to guide people into a better life style
which help individual to develop better living. Ayushman Bharat is an attempt to move from selective
approach to healthcare to deliver comprehensive range of services i.e. preventive palliative,
rehabilitative, etc. It has 2 components which are complementary to each other :-
1st component – 150,000 (Health wellness centre)
2nd component is PMJAY (Pradhan Mantri Jan Arogya Yojna) which provide health insurance cover of Rs.
5 lakh per year over 10 crore poor & vulnerable families for seeking 20 to 30 care.
By 29th Dec 2022 – 1.5 lakh Health & wellness centre are in India (functional HWCs)

The delivery of CPHC through HWCs involve is complex task as it requires a paradigm shift at all level of

“THE FIRST HWC WAS LAUNCHED IN BIJAPUR, (C.G.) ON 18TH APRIL, 2018. IN THE 1ST
YEAR OVER 17000 HWCs WERE OPERATIONALIZED, MORE THAN THE TARGET OF 15000
SET FOR FY 2018-2019”
ORIENTATION REPORT:
On 12/12/2022 @ 10 AM
❖ MSc students & 4th year students reached the PHC Kohka. We all met with our teachers and
they orient us about the hospital. We enter through main gate :-
❖ Waiting area in front with full facility of sitting arrangements for the people visiting PHC with full
facilitate ventilation and lightning.
❖ In our left as shown in map of Kohka PHC: - Room no. 18 (Vaccination Room) where 2 sisters
(S/N) are available they are taking vitals of patient & send them to OPD.
❖ Room no. 10 (Eye Dept.) ophthalmic assistant room is there, they select the cases for cataract
operation & diagnose the patient for any opthal related problem & send them to higher facility.
❖ Dressing area outside the vaccination room there is dressing area with proper BMW
management for disposable of waste.
❖ Pathology room 1 lab technician is there for testing the sample & proper investigation room is
there with proper arrangements of old withdraw facilities & sitting arrangements.
❖ Male ward 3 beds capacity with proper ventilation & proper toilet facilities, 3 IV stands, proper
lightning, BMW management small Nsg station.
❖ Feeding Room proper closed room available for feeding room with proper arrangements &
ventilation.
❖ Female ward having 3 bed capacity, proper toilet facility, 2 IV stands, bed side locker, cooler,
proper facility of lights & ventilation.
❖ Store room closed in PHC Kohka, but it is used to store extra & essential material required in
PHC.
❖ Labour room includes 2 delivery table capacity, 1 radiant warmer, 1 suction apparatus, 2 O2
cylinder (1 manual & 1 electrical) 1 turbo O2 cylinder without maux regulator, 1 manual BP
apparatus, 1 OT light, 2 IV stand, 1 boiles, 1 trolly, 1 baby wt machine, 1 autocare Machine, with
full inverter, 1 needle cutter, proper BMW management, Hand Washing Area & toilet exhaust,
proper ventilation & lightning 1 functional thermometer is placed which work to maintain the
normal temperature between 25oC to 28oC of the room temperature of labour room
maintained.
7 trays are present in labour room in a sterile way
includes baby tray, emergency tray, delivery tray, episiotomy tray, medicine tray, PPIUCD tray,
MVA tray.
IN RIGHT SIDE FROM WAITING HALL: -

❖ Pharmacy room: - where proper and latest medicines are available before expiry day &
expiry date is mentioned clearly.
