We are currently in the process of setting up an overseas land-based recruitment agency called Oriental
Service Manpower, Inc and are working on getting all the pertinent documents to support our
applications for the issuance of provisional license.

Part of the checklist of requirements for issuance of license is securing the Anti-Illegal
Recruitment Branch (AIRB) clearance(s), valid for at least (6) months from your department
(Ref: Par. 7: FM-POEA-03-IR-01A).

I have attached here the accomplished Migrant Workers Protection Bureau Multi-Purpose clearance
form and all the supporting documents (in JPEG format) for your review and consideration.
