B. SEC(Securities and Exchange Commission) or DTI(Department of Trade and Industry)

Registration
1. May require TESDA and LTO endorsement prior to filing

C. LGU Permit(Mayor’s Permit)

1. Requires SEC/DTI Registration

D. BIR COR(Bureau of Internal Revenue Certificate of Registration)

1. Requires SEC/DTI Registration and LGU Permit

E. List of accredited instructors

At least 1 theoretical and 1 practical driving instructor.
1. Accreditation Requirements of Driving School Instructors
a. Certified true copy of diploma and appropriate certificates of training
b. Copy of valid driver’s license
c. Original neuropsychiatric test result
d. NBI/Police clearance
e. Drug free testing certificate
2. Instructors must be affiliated to only one Driving Institution,

F. Location plan and a sketch of maneuvering site (with lease agreement if NOT owned)

G. Pictures of Driving School/Training Center (Interior and Exterior)

H. List of motor vehicles registered under the name of the Driving School/Center exclusively
used for a practical driving lesson
1. Supplemental IRR of RA 10930 compliant
2. Driving Schools may opt to operate PDC for either Motorcycle or Light Vehicles or
both

J. Sworn statement with documentary evidence of the adequacy of funds for annual
operating expenses

K. Bank Certificate
1. For Driving School: Bank Certificate/Credit line worth P3,000,000 under the account
name of the Driving School
2. For DEP(Driving Enhancement Program): Bank Certificate/Credit line worth P1,000,000
