JUDICIAL AFFIDAVIT

I, Michael Jordan Dela Cruz, Filipino, of legal age, with address at 2324
Kasaganahan St. Brgy. Paltok, Quezon City, after being duly sworn to in accordance
with law, depose and say:

I. PERSONAL CIRCUMSTANCES OF THE AFFIANT

I am of legal age, single/married, and a resident of 2324 Kasaganahan St. Brgy.
Paltok, Quezon City;

I am a Licenced Teacher , currently employed at Mabini Elementary School, with

business address at 2005 Mabini St. Brgy Paltok, Quezon City;
I am executing this affidavit to attest to the truth of the facts herein stated and to
be used in to prove that the defendant has already settled his due obligation to
the plaintiff.

II. NARRATION OF FACTS

On April 2, 2022, at around 10:00 am, I was CezarLands’ principal office at 999
Continental Bldg. West Avenue, Quezon City;
I handed the postdated check to Ms. Anna Manalastas, in reference to my account
and due obligation amounting to 12,000.00;
That Ms. Anna Manalastas received the check, and allowed me to sign the paper
in reference to the payment of the amortization;
That Ms. Anna Manalastas gave me a copy of the receipt;
I am executing this affidavit to attest to the truth of the foregoing facts.s

III. JUDICIAL AFFIDAVIT PROPER

1. Q: Do you swear to tell the truth and nothing but the truth?
A: Yes, I do.
2. Are you really sure that it was Ms. Anna Manalastas, the account officer, that
you gave the postdated check?
A. Yes Sir.
3. How did you know?
A. She was wearing an Identification Card po with the lace/lanyard of the
company.
IV. CONCLUSION

In view of the foregoing, I am executing this affidavit to attest to the truth of the
foregoing facts and for such other legal purposes as it may serve.
