This is an interactive form and applies to a person making application to acquire and possess ammunition for the purpose of
an ammunition collection only.
Please complete all sections, print this form and submit with the P634 'Application for a Firearms Permit' form and any
supporting documentation to the Firearms Registry, Locked Bag 5102, Parramatta NSW 2124.

1. EVIDENCE OF LEGITIMATE REASON FOR AMMUNITION COLLECTION


The Commissioner must not issue a permit unless satisfied that the applicant has a legitimate reason for the permit.
Provide information below on the nature and purpose of your ammunition collection.
I am a financial member of a ammunition and firearms collectors' society/club. My ammunition and firearms collection is
thematical, financial and historcial and I have supplied a copy of my Sporting Shooters Association of Australia membership.

2. DETAILS OF THE TYPES OF AMMUNITION IN THE COLLECTION


Provide information below on the types of ammunition in your collection or ammunition that you wish to collect.
NOTE: The ammunition must not contain high explosive, smoke, chemical or lachrymatory properties or agents.
Conventional small arms ammunition (rimfire, centrefire, rifle, pistol and shotgun ammunition).

3. CERTIFICATION THAT AMMUNITION HAS BEEN RENDERED INERT


You must complete the following section to certify that all ammunition forming part of the collection has been rendered inert,
except UN Hazard Classification Code 1.4.S up to 20mm calibre.

• I hereby declare that all ammunition forming part of the collection is or will be rendered inert, other than any sporting or
military ammunition of UN Hazard Classification Code 1.4.S up to 20mm calibre.

4. AMMUNITION SAFE KEEPING FACILITIES
All ammunition must be stored in a locked container, locked display case or other locked cabinet which is sturdy in
construction and not easily penetrated. Provide information below on your arrangements for safe keeping and storage of
ammunition.

FURTHER INFORMATION
It is a serious offence under the Firearms Act 1996 to make a statement or provide information in relation to an application that
is false or misleading. By signing the Declaration on your Firearms Permit application form, you are certifying that ALL the
information supplied in relation to this application is true and correct.

INDIVIDUAL APPLICATION
DO NOT SEND PAYMENT WITH THIS APPLICATION -If approved you will be sent a photographic advice to attend the RTA to
have your permit card issued and you will pay at the RTA.

BUSINESS APPLICATION
YOU MUST SEND PAYMENT OF $75 WITH THIS APPLICATION. IF PERSONS OTHER THAN THE NOMINATED PERMIT HOLDER
ARE LISTED ON THIS PERMIT, A P635 FORM MUST BE COMPLETED FOR EACH PERSON AND FORWARDED WITH THIS
APPLICATION.

CHECKLIST - Make sure you have supplied ALL of the following required information
All applicants are required to provide:

A completed P634 'Application for a Firearms Permit' form

Proof of Identity (if no other NSW firearms licence or permit is held or has been held).

A Legitimate Reason form with the relevant sections completed:

Evidence of legitimate reason for permit

Details of the types of ammunition in the collection

Certification that all ammunition has been rendered inert (except UN Classification Code 1.4S up to 20mm calibre)

Ammunition safe keeping facilities

Individual Application - DO NOT SEND PAYMENT WITH THIS APPLICATION


If approved you will be sent a photographic advice to attend the RTA to have your permit card issued.

Business, Club, Organisation - Fee $75 (cheque, money order or credit card authority), with the application

BUSINESS APPLICANTS ONLY - A completed P635 for each employee to be authorised by this permit AND proof of identity
for each employee (if no other NSW firearms licence or permit is held or has been held).
