of Disease Prevention and Control.

Visitor Open Time

The vaccination services at Yogyakarta Class III Port Health Office (KKP) are meningitis and
yellow fever vaccines.

Vaccination registration is open every weekday

MONDAY - FRIDAY = 08.00 to 12.00 WIB

The vaccination requirements that should be brought are:

1. Online registration form


2. Photocopy of Passport as a 1 (one) sheet or the passport file has been uploaded on the online
registration form

3. Photocopy of KTP / KK (for those who do not have KTP) as many as 1 (one) sheet

4. Colored photo 4x6 size = 1 (one) sheet.

PROFILE
History and Overview
Yogyakarta Class III Port Health Office (PHO) is a Technical Implementation Unit (TIU) of the Ministry
of Health of the Republic of Indonesia which is organized under the authority of Directorate General of
Disease Prevention and Control. Class III Yogyakarta PHO was originated from the expansion of PHO
Class II Semarang. On May 7, 2012, the Director General of Disease Control and Environmental Health
(PP and PL), Ministry of Health, Prof. dr. Tjandra Yoga Aditama SpP(K), MARS, DTM&H, DTCE
inaugurated the Yogyakarta Class III PHO as the 49th PHO in Indonesia. The Yogyakarta Class III PHO
was formed based on the Decree of the Minister of Health of the Republic of Indonesia Number
2348/Menkes/Per/IX/2011 concerning Amendments to Permenkes Number 356/Menkes/Per/IV/2008
concerning the Organization and Work Procedure of the Port Health Office.

Yogyakarta Porth Health Office is located at Jalan Ringroad Utara No. 8 Nanggulan Maguwoharjo, Depok,
Sleman DIY was built on state-owned land with an area of 492 m2 and an initial building area of 100 m2.
At the moment, the Yogyakarta Porth Health Office has completed a total rehabilitation with a building
area of 912 m2 with the initial condition that all activities, both service and office operations, were
accommodated in a one-story building, now it has become a four-story building. This rehabilitation aims
to increase convenience in vaccination services to the user community as well as the smooth running of
office operations. And the new Yogyakarta PHO building was inaugurated on May 2, 2017, by the Director
General of Disease Prevention and Control of the Indonesian Ministry of Health, Dr. H. Mohamad Subuh,
MPPM.

At the moment, the meningitis vaccination service is the largest contributor to the Non-Tax State Revenue
(NNSR) of all types of NNSR in the Yogyakarta PHO. The Yogyakarta PHO issues an International
Vaccination Certificate for every vaccine administered, or better known as the ICV (International
Certificate of Vaccination) book. This is in accordance with article 8 (2) of the Minister of Health No. 13
of 2016 that International Vaccination Certificates are issued by the PHO, or the hospital appointed by the
minister. What is meant by "International Certificate of Vaccination" is evidence that a person has received
the necessary vaccinations and/or prophylaxis for certain international trips.

In Article 2 (1) Permenkes No 13 of 2016 that vaccination is carried out by everyone who travels
internationally from and to countries infected and/or endemic for certain infectious diseases and/or at the
request of the destination country must be given certain vaccinations in accordance with the provisions of
laws and regulations. And according to these regulations, meningitis vaccination services can only be
carried out at KKPs that have been established throughout Indonesia and at hospitals appointed by the
Minister. As for the appointment of a hospital to carry out meningitis vaccination activities for Umrah
pilgrims according to the Decree of the Minister of Health of the Republic of Indonesia Number
026/Menkes/SK/II/2014 there are six hospitals in Indonesia namely Adam Malik Hospital Medan,
Fatmawati Hospital Jakarta, Jakarta Friendship Hospital, Jakarta National Brain Hospital, Dr. Wahidin
Sudirohusodo Hospital in Makassar and Ulin Hospital in Banjarmasin.

In addition to meningitis and yellow fever vaccination services as contributors to PNBP, there are also
services for issuing health quarantine documents at Yogyakarta Adisutjipto International Airport such as
permits for transporting bodies, permits for the transport of sick people and babies, certificates of
flightworthiness for pregnant women, health certificates, drug transport permits, food, cosmetics, medical
devices, and addictive substances. PHO Class III Yogyakarta has two working areas, namely Baron Seaport
and Kulon Progo Seaport. Vaccination service activities in work areas are also supported by doctors and
nurses aiming to reach meningitis vaccination services in the area.

Main Duties and Functions


Duties:

Port Health Office Class III Yogyakarta has duties for carrying out prevention of disease spread, potential
disease outbreaks, epidemiological surveillance, quarantine, environmental health impact control,
OMKABA supervision and safeguards against new diseases and re-emerging diseases, bioterrorism,
biological elements, chemical and radiation protection in the working area of airports, ports and national
land crossings. (ARTICLE 2 OF REGULATION OF THE MINISTER OF HEALTH OF THE REPUBLIC
OF INDONESIA NO. 356/MENKES/PER/IV/2008 ON ORGANIZATION AND WORK OF PORT
HEALTH OFFICE).

Functions:

In carrying out the tasks referred to in Article 2, Port Health Office carries out 16 (sixteen) functions (Article
3 of REGULATION OF THE MINISTER OF HEALTH OF THE REPUBLIC OF INDONESIA NO.
356/MENKES/PER/IV/2008 ON ORGANIZATION AND WORK OF PORT HEALTH OFFICE):

1. implementation of quarantine;
2. implementation of health services;
3. implementation of environmental risk control at airports, seaports and national land crossings;
4. implementation of disease surveillance, potential disease outbreaks, new diseases, and
diseases that reappear;
5. implementation of ionizing and non-ionizing, biological, and chemical radiation protection
6. implementation of epidemiological surveillance network centers/nodes according to diseases
related to national, regional, and international traffic;
7. implementation, facilitation, and advocacy of preparedness and handling of
Extraordinary Occurrence (KLB) and disasters in the health sector, as well as dimensional
health including haj health management and population migration.
8. implementation, facilitation, and advocacy of occupational health in the environment of
airports, ports, and national land border crossings;
9. implementation of issuing health certificates for export drugs products, food, cosmetics and
traditional medicine (OMKABA);
10. implementation of health supervision of means of transportation and cargo;
11. implementation of the provision of health services in working areas of airports, seaports and
national land crossings;
12. implementation of information and technology networks in the health sector at airports, ports,
and across national land borders;
13. implementation of networks and partnerships in the health sector at airports, seaports and
national land borders;
14. implementation of quarantine studies, environmental risk control, and port health surveillance;
15. implementation of technical training in the field of health at airports, seaports and national
land crossings;
16. implementation of Port Health Office administration and housekeeping.

About Us
Development of Health in Port/Airport Areas is part of the development of National Health.
Currently, ports/airports do not only function as entry points for goods or services and people, but
can also be as entry and exit points for quarantine diseases and potentially infectious diseases.

Yogyakarta Class IV Port Health Office (KKP) is domiciled in D.I Yogyakarta Province,
is one of the UPTs of the Ministry of Health under the Directorate General of Disease Prevention
and Control in the Province of D.I. Yogyakarta, besides BBTKL-PP Yogyakarta. The scope of the
Yogyakarta Class IV KKP work area covers the entire Province of D.I Yogyakarta. Yogyakarta
Class IV KKP is located to the east of the city of Yogyakarta with coordinates 7046'S and 110025'E
with a height of 206 meters above mean sea level (MSL = Mean Sea Level). The Yogyakarta Class
IV KKP Main Office is located in Maguwoharjo Village, Depok District, Sleman Regency,
Yogyakarta City. Where the Main Office is responsible for the traffic of sick patients at Yogyakarta
Adisutjipto International Airport

The Port Health Office (KKP) is a technical implementation unit within the Ministry of
Health that is technically and administratively responsible to the Director General of Disease
Prevention and Health (Ditjen P2P). KKP has the main task and function of preventing the entry
and exit of quarantine diseases and potential disease outbreaks through actions without hampering
travel and trade.

Yogyakarta Class IV Port Health Office has 2 (two) working areas, namely the Working
Area of Kulonprogo Seaport and Baron Seaport. The Port Health Office has the task of carrying
out the prevention of entry and exit of quarantine diseases and potential infectious diseases through
aircraft, maintenance and improvement of aircraft, and airport environment sanitation as well as
limited health services at airports based on applicable laws and regulations.
Based on the Minister of Health of the Republic of Indonesia Number:
2348/Menkes/Per/XI/2011 dated 22 November 2011 the Yogyakarta Port Health Office is
classified as a Class IV Port Health Office whose main tasks and functions consist of
Administrative affairs, Quarantine Control Officers and Epidemiological Surveillance, Risk
Control Officers Environment and Health across Regions, Installations, Work Areas and Groups
of Functional Positions. The duties and functions of the Yogyakarta Class IV Port Health Office
are to carry out prevention of entry and exit of diseases, potential disease outbreaks,
epidemiological surveillance, quarantine, environmental health impact control, health services,
OMKABA supervision and safeguards against new diseases and re-emerging diseases,
bioterrorism, biological elements, chemical and radiation protection in the working area of
Airports, Ports and National Land Border Crossings.

This Regulation of the Minister of Health is the implementation of the International Health
Regulation (IHR), in all Ports/Airports of UN Member countries that ratify it, intending to detect
and respond in preventing the entry and exit of PHEIC and bioterrorist diseases through
International ports.
