Standard Operating Practices

Purpose and Scope


To provide guidance in the event of navigational equipment failure. This applies to all
vessels.

Responsibilities
Master
 Ensure that all watchkeeping officers understand the emergency procedure to be
followed in case of navigational equipment failure at sea.

 Take appropriate action to ensure the safety of the ship, considering all
prevailing circumstances, such as traffic, proximity of land or isolated danger,
weather / sea conditions.
Bridge Watchkeeping Officer

 Immediately inform the Master of the failure.

 Obtain and plot the ship’s latest position on the chart /ENC. Monitor traffic and
proximity to nearest navigational hazards.

Notification
The Master provides notification to the following:

 As applicable, broadcast a warning to all ships in the vicinity. Notify the local
coast guard / vessel traffic services (VTS) if appropriate.

 Advise Teekay management as per First Alert Procedure (SP0268N) / First


Notification Procedure (SP2300N).

 Ensure required Local Authority / Class / Flag notification are carried out in
consultation with Ship Team.

 Record the failure as unplanned maintenance for fleet wide trending of


navigation equipment failure.

Training
Training is essential to prepare ship staff for various Navigational equipment failure.
Practical drills, as described on the following, must be conducted at regular intervals:

 Operations in the Case of Bridge Equipment Failure (FM1324N)

 ECDIS Failure Checklist mentioned in Navigation Hand Book (Chapter 10)

Response to Various Navigational Equipment Failure


When functionality of equipment is fully restored:

 Notify all concerned authorities, Class & Flag as applicable.

 Update and close the Unplanned maintenance in PMS.
