Removal and Installation

What really happened…


Case 1: Flight Cut Short

5 minutes after takeoff, the flight crew got a #2 engine Low Oil Pressure warning. When the
pressure reached 9 psi, the crew secured the engine and continued to the destination. An
uneventful single engine landing was performed. Maintenance found the engine cowl loaded
with oil and the Accessory Gear Box borescope crank pad plug was found off. Maintenance
records indicate that the engine had been borescope inspected for AGB distress prior to the
event flight. The crew that performed the BSI stated that they did install the plug. Maintenance
noted the plug was off, but the snap ring was not found.

Case 2: Hanging On by the Lanyard

Upon descent, a pilot reported a #1 engine Low Oil Pressure warning. The crew followed
procedure and shut down the engine. An uneventful single engine landing followed. Upon
inspection, the engine crank pad cover was found hanging by its lanyard.
