PROBATIONARY EMPLOYMENT CONTRACT

(NAME OF EMPLOYEE), we are pleased to inform you that your application

as a [____________] for our company PhysioAve Physical Therapy Services has
been accepted. We would like to offer you a probationary employment for six (6)
months commencing on [date] up to [date], under the following terms and
conditions:

1. As a probationary employee you are expected:

a. To exhibit commitment and passion for work;

b. To be honest and have integrity as a member of our company;
c. To fully cooperate and maintain a healthy relationship with management and
your co-workers;
d. To promptly report for work at exactly _______ until ______.; (this provision
is amenable for negotiation) at PhysioAve Physical Therapy Services –
Daan Sarile, Cabanatuan City, Nueva Ecija and its subsidiaries, both
home and clinic services;
e. Within a month, you are expected to report for work five (5) days a week, for
two (2) weeks (80 hours) and Six (6) days a week including Saturdays for the
other 2 weeks (96 hours). Rest days can be offset to other days of the
week in cases where services are needed to be done during rest days;

2. The monthly salary should be _______________. However, we impose a “No

Work, No Pay” policy;

3. In compliance with the law, the company shall deduct from your compensation
all necessary contributions to SSS and Philhealth including Health
Maintenance Organization (HMO) shall likewise be made and remitted to the
proper agencies concerned;

4. Commissions shall only accrue after you have handled a number of forty (40)
patients, in accordance with the table below, to wit;

1-40 patients – 0% of the payments of the patients;

41-60 patients – 5% of the payments of the patients;
61-80 patients – 10% of the payments of the patients;

The amount of commission shall be subject to increase or reduction

depending on the monthly income of the clinic;

5. The company allows receipt of tips from clients. This does not form part of
your salary and will not be included in your reported compensation;

You are prohibited to use or disclose to any other persons or entity any
confidential information which you may have obtained access to during
employment. Further, in order to ensure the clinic’s confidences, you
are prohibited from being employed with any other related business
during the term of your contract, you are also prohibited from accepting
schedules and therapies from the patients of the clinic. The EMPLOYEE
shall treat as confidential all information such as but not limited to, those
concerning matters affecting or relating to the EMPLOYER’s business and
operations, plans, processes, techniques, technology, trade secrets, and other
confidential or sensitive matters covering EMPLOYER’s services or any of its
affiliates and clients. The same shall not be used for personal gain and
shall remain confidential even after the end of this contract. The
disclosure and use of the same for personal gain shall be punishable
under Intellectual Property Act and other applicable penal laws. In
addition thereto, any violation of this AGREEMENT shall carry with it a

 liquidated damage equivalent of Php 40,000.00, or the remaining month’s
salary, whichever is higher;

6. During the probationary period of your employment, your performance shall be

reviewed and evaluated in accordance with this contract and the company’s
standards in order to determine your fitness and qualifications. It is a must
that during the entire period of your employment, you abide by all existing
company policies, written or unwritten as well as its rules and regulations;

7. The company reserves the right to terminate your services at any time within
this probationary period in the event that your performance is deemed
unsatisfactory or for any justifiable or authorized causes in accordance with
law;
