PRIMARY CONCERN FOR WELLBEING FORM

At the Imperium Entertainment Bureau, your health or mental health needs are very
much validated. We understand that set work is to be considered with utmost priority, but
the I.E.B. platform will never allow any talent to compromise their health or safety
standards for the sake of any role. All measures of safety will be taken to make sure that
talent, in addition to crew members, stunt teams or coordinators, will be protected from
any life threatening situations.

(If comfortable) talent can disclose any pre-existing health or mental health limitations
through the I.E.B. health form provided prior to principal photography.

We do not wish for any of our material to further induce or exacerbate any existing
physical or mental health conditions or shortcomings. Your health and wellbeing should
be considered a primary priority above any entertainment opportunity.

Please be aware that if you or any talent present during principal photography
experiences a mental breakdown, panic attack or sudden decline, filming will be paused
and we will immediately proceed to getting talent to a temporary safe place for their own
and loved one’s safety and convenience. Emergency or ambulance may be called for
severe breakdowns or panic attacks.

At the I.E.B platform we do not want any talent or crewmember to ever feel any isolation
or self-conscious feelings during difficult physical or mental health circumstances. The
founder of this company deeply resonates with trauma and affliction done in and out of
private matters. Once again, your health and mental health needs are considered a top
priority at the platform.
 CHILD ENDANGERMENT & WELFARE CONCERN

Once again, a child’s safety and wellbeing are a top concern at the Imperium
Entertainment Bureau. Underaged persons will NEVER take part in the following for the
sake of safety against child endangerment & sexual abuse:

● Highly explicit themes or material depicting underaged persons or minors in

public or implied nudity settings.
● Material depicting underaged persons or minors using explicit or vulgar language.
● Material depicting minors using heavy drug or alcohol use.
● Material depicting minors in sexual or perverted settings.
● Material depicting minors in explicit or graphic violent scenarios.

Parents must be aware of present dangers posed to children in today's world and
media. We can not guarantee that all film settings will be safe outside of the Imperium
Entertainment Platform. NEVER leave or allow your children to be alone with
producers, agents, adult talent & filmmakers. We kindly ask you to continue to keep
your children safe, alert and near you while helping them embark on their talent or film
career(s).
