White Background 2 x 2 picture
Properly filled-out Health Record Form - Medical & Dental

For Transferee Only - Original Transfer Credentials/Honorable Dismissal from previous school (Certificate of GoodMoral
Character, Scholastic Records & Transcript of Records).

*Documents should be enclosed in a long brown envelop with your full name at the upper left
corner (front). Students with incomplete requirements will not be
processed and will be notified thru email.
