Pleasant Days Resort - Incident Report Form

Pleasant Days Resort policy is to require all hotels to report all injuries, accidents, losses, damage, fires, any other
incident that could result in a claim against the Company.

This form must be used to report all incidents and must be completed and emailed to concerned department
head and copied to resident manager.

I got call from Maharaj front office department at 00.50 AM in room no 404 & 408 extra people are staying in the
room. My self and maharaja and other staff went to Room No 404 and rang the bell. Guest came out we found
one lady and 3 other person extra apparat from Staying guest. We informed the guest as per our policy only two
guest should stay but 4 people are staying extra in the room with a single lady. We ask them to check out of
hotel. 3 of them fully drunk and speak without manace. Lady was shouting at us why so much people waiting out
side of the room. We asked other staff to leave the place myself and Maharaja handling the issue. One guest hit
Maharaja lightly in face and we ask them to kindly leave the resort in 15 Minutes. After that at 01.40 Am guest
left the hotel premises. Morning Guest checked out at 9.30Am and settled all the bills.

Enquiry

Myself checked with security how the guest came inside the hotel and went to room. Satyam enquired the guest
where they are going inside the hotel. The guest told going to Restaurant. After that I checked the information in
front office about guest in the record. I requested the housekeeping supervisor to check the floor then and there
to confirm Guest is not checked out. Because they have due amount to pay from room server order. Morning at
9.30am guest checked out and all the bill has been settled
