Brief about the drill, Extenuating Circumstances / Identified Factors / Special Conditions Simulated:
Exactly 14:18PM the drill has started.The main objective of the drill is Falling From Height Rescue.

One of our Rigger Pankaj Kumar got slipped and fell down during descending from ladder (1.7mtr height)
at TF1 area, immediately the co worker Lalu A. B shouted and passes the information to the site supervisor
& site Safety Steward 14:19pm.The Safety Steward informed by phone on 14:19PM to our first Aider &
informed by phone on 14:20PM to our work incident controller team.

The following sequence of events noted during the Mock Drill:

14:22 The First Aider arrived at site with first aid box & gave First Aid to the victim.
14:23 Work incident controller (Misc. Tuaman) reached at Incident spot with Emergency vehicle.
14:23 EIL HSE team reached at site.
14:24 The victim shifted to the Emergency vehicle & left to the hospital.
14:25 All the activity was stopped & informed all to assembly at the designated assembly point.
14:27 Head count done.
14:29 Briefly described all regarding the incident & about Emergency Preparedness by HSEO.
14:37 Briefly described about Emergency Preparedness by TEL Management & EIL HSE Team.
14:44 Informed to all that fall from height mock drill conducted.
14:45 Declared all clear & workers returned to work.
Details of drawbacks:
1. Lack of traffic control has identified at the time.
2. The victim was not using grab arrester during descending from ladder.

Corrective Action :

1. Awareness training given to all workers regarding the importance of using of grab arrester during
ladder climbing & descending.
2. Periodical mock drill training required to eliminate the above drawbacks.
