Section A: Introduction

1. Internal Environment

Rafael Lazatin Memorial Medical Center is a hospital which includes pharmacy


services that is licensed to sell drug products, including temperature sensitive
products and controlled substances.

Rafael Lazatin Memorial Medical Center license number 03-0172-23-H1-1 located at


Visitacion Street, Villa Rosario Pampang, Angeles City, Pampanga 2009

Rafael Lazatin Memorial Medical Center is owned by the City Government of


Angeles.

The following the responsibilities of the pharmacist:

• The establishment is open round-the-clock under the supervision of a duly licensed


pharmacist.
• Assure the compliance with all legal and regulatory requirements.
• Ensure availability of affordable, quality, safe, and efficacious medicines/drug
products.
• Maintain the good condition of the storeroom, manage the storage of drug products

2. Risk Management Approach

The establishment will use the SOP (SOP No. 1-18). The RMP will be reviewed 3
months before the expiration of the LTO. Minutes of the meeting will be taken.
