NETWORK: FIRE REPORT


PURPOSE: For reporting of ALL FIRES attended by Essential Energy Employees especially where
Essential Energy assets have been damaged and/or the alleged cause of ignition involved Essential
Energy or Private Line Assets. This form is to be used by Field Employees to convey information to
the Regional/Area Resource Person who creates the TotalSAFE log and then attaches the report in
the Incident details section.

Priority is always to make safe and restore supply then reporting. It should be noted that
normally some verbal reporting takes place soon after the event with formal reports such as this to
be issued as soon as time permits, shortly after the event. Responsibility for forwarding reports is
indicated above.
