Subject: LETTER OF AUTHORIZATION WITH WAIVER & ASSUMPTON OF RISK

To Whom It May Concern:

I/We acknowledge that the East West Banking Corporation (“EWBC”) standard procedure for the release
of collateral documents is that I/We or my/our duly authorized representative should personally pick up
such collateral documents from your office at the EastWest Corporate Center located at the 6th Floor
EastWest Corporate Center, The Beaufort, 5th Ave. Cor. 23rd St., Bonifacio Global City, Taguig City. I further
acknowledge that such procedure is for my protection. This notwithstanding, I/We have nevertheless
authorized _______________________, _____________________ (“Nominee”) of East West Bank’s
__________________ Store (“Preferred Store”), to claim, on my behalf, from your good office my
collateral documents in relation to _____________________ described as follows (hereinafter,
“Collateral Documents”) and to cause the transmittal of such Collateral Documents to my Preferred Store.

I have also attached photocopies of my 2 valid ID’s for identification & validation purposes, which ID’s are

I understand and agree that the release of my Collateral Documents to my Nominee and/or the transmittal
of my Collateral Documents from your good office to my Preferred Store is attended with risks such as but
not limited to the loss and/or damage to as well as the unlawful use of the Collateral Documents. In view
of this, I hereby declare that I am assuming all risks associated with my foregoing instruction to cause the
release of my Collateral Documents to my said Nominee and/or its transmittal to my Preferred Store. I
also agree to unconditionally and absolutely hold EastWest Bank, its employees, and officers free from
any and all liabilities, losses, claims, actions, proceedings, demands, damages, costs and expenses that
I/We may suffer out of or in connection with my/our instruction to release my/our Collateral Documents
to my/our Nominee and/or its transmittal to my Preferred Store. I/We likewise waive any action against
EastWest Bank arising out of or in connection with the release of my/our Collateral Documents to my
Nominee and its transmittal to my Preferred Store.
