PRE-EMPLOYMENT UNDERTAKING

(Macau Candidates)

I, _________________________________ of legal age and a resident of

_________________________________________________________ do hereby acknowledge
and undertake to comply with the following:

1. I am aware that upon signing the offer letter, I have been selected by
_______________________________________ (hereinafter referred to as the “Employer”),
through Ikon Solutions, Asia, Inc. (hereinafter referred as “Ikon”), to be employed as
___________________;

2. I fully understand the employment package and other benefits offered by the Employer and all the
terms stated in the offer letter;

3. As I may be allowed to initially submit all the needed requirements/documents through email to
start the deployment process, I agree and commit to submit it within the prescribed period and to
represent and warrant that the documents I will submit, including information stated in my
resume, are genuine and truthful; and that the data I furnish are all true and correct. If proven
that I have falsified or misrepresented myself in any document submitted to Ikon or my Employer,
Ikon may file a case against me with the Department of Migrant Workers (DMW) or any other
court that has jurisdiction over the subject matter.

4. If I will be required to submit the original documents through private courier, Ikon assumes no
liability or responsibility for the consequences arising out of the delay and/or lost in transit of such
original documents.

5. The offer letter is enforceable only if I complete and pass all the formalities including but not
limited to the medical test, technical test (if applicable), approval of license (if applicable), work
permit, BACKGROUND CHECKING of previous employers, criminal records, and other
required examinations and verifications.

6. Any expenses or costs incurred in completing or processing the test or verification mentioned in
paragraph no. 5 will not be subject to reimbursement. Ikon is also not responsible for any claims
or damages sustained by reason of the processing of my employment application including, but
not limited to, immediate resignation from current work or obtaining a loan from a 3rd party;

7. I understand that if I decide to cancel, withdraw my application, and/or failed to comply with the
requirements for whatever reason after a fit to work certificate is issued by an accredited clinic,
Ikon has the right to collect from me a penalty of TEN THOUSAND PESOS (Php10,000.00) to
cover the liquidated damages and other administrative expenses incurred in handling my
application.
Furthermore, if the employer has already obtained a WORKING VISA under my name and/or
AIR TICKET, the actual cost of the same including liquidated damages in the amount of FIFTY
THOUSAND PESOS (Php50,000) will be charged to me of which I agree to pay.

8. Non-compliance with the immediately preceding provision which may result in further damage to
Ikon’s relationship with the Employer shall cause Ikon to file a complaint or a disciplinary action
 against me with the DMW that will disqualify me from deployment to any other countries and/or
file an appropriate case before the regular courts for claim of damages.

9. In the event the Employer withdraws the job offer, for reasons unknown to and not participated
in by Ikon, I agree not to hold Ikon liable for any damages after clear presentation of
proof/justification to the cause of withdrawal;

10. I hereby confirm that I have been directed by my employer and agency not to resign from my
present position (if employed) until after I passed the background check and I have been informed
that I am finally selected for the position that I applied for. Should I decide to resign on my own,
I will not hold my agency and employer for this decision;

11. I have no pending civil, criminal, or administrative cases before any court in the Philippines and I
have not been convicted of any offense or crime involving moral turpitude;

12. For female, I hereby confirm that I am negative of pregnancy test upon the submission of my
application form; and will continue until my eventual deployment and employment. Any request
of cost reimbursement by reason of pregnancy will not be granted and maybe subject to damages.

13. I further commit that I will not directly contact my prospective Employer before formal hiring or
deployment;

14. I declare that all above stated items/provisions have been carefully explained to me and that I have
signed and executed this agreement on my own free will and with full knowledge and
understanding of the same and its consequence.
