AFFIDAVIT

  I, JOSE FRANCISO ORTEGA, of legal age, Filipino citizen, widower and resident of
Gairan, Bogo City, Cebu, Philippines, after having been duly sworn to in accordance
with law, depose and state:

1. That I am the sole legal heir of the deceased ERLINDA TEQUILLO

ORTEGA;

2. That in the Affidavit of Self Adjudication acknowledged before Notary Public Reynaldo
A. Ornopia; bearing Doc. No. 162; Page No. 34; Book No. 1; Series of 2020 that the
statement below was not indicated:

“That the provisions of Article 1623 of the New Civil Code of the Phil. have been previously
complied with.”

“That the above-described property, not having been registered under ACT 496 nor under
the Spanish Mortgage Law, the parties hereto have agreed to register this instrument under
the provisions of Sec. 194 of the Revised Administrative Code, as amended by Act 3344.”

3. That I am executing this Affidavit to attest the facts and to use the same for
whatever legal purpose it may serve.
