to the Dampproofing Polyethylene Sheet 250 Microns by M/s Fast International Petrochemicals
Co. Ltd (DADCO), please be advised that the submittal has been reviewed and given the status "B"
Approved as Noted with the following comments:

Consultant's Comments:

1. The general organization for social insurance and ministry of labor certificates have to be
renewed and valid prior to use of the proposed material.

This approval is subject to the compliance from the above requirements and does not relieve the
Contractor of his obligations under the Contract and he shall be liable for all acts, defaults and
negligence by the subcontractor.
