1. That I am the owner of a business establishment named BUCAD FOOD AND BEVERAGE
STORE located at Ground Floor , Red Hotel Cubao # 627 Pinatubo St. Brgy. San Martin De
Porres Cubao Quezon City.

2. That I have been issued a DTI Permit No. 1254345 valid from October 22, 2019 to October 22,
2024.

3. That on July 19, 2023,I shall close the said business due to mismanagement.

4. That I execute the foregoing instrument in order to attest the truth of the above facts for all
legal intents and purposes it may serve.
